摘要
涤纶网硅胶板单面扩张器（ＥＰＳ）在颈部的应用：增扩皮肤软组织的面积，以修复双面部皮肤的缺损；减少传统皮肤扩张器时对深部压力所致并发症。方法采用涤纶网硅胶板单面皮肤扩张器２５０～３００ｍｌ容量，埋置于颈阔肌深层，充分分离腔隙，将单面扩张器置于颈阔肌深层。术中即注水８０ｍｌ左右，形成内压，减少出血，剥离面内置引流，阀壶置体外定期注水。经过３２例临床总结，涤纶网硅胶板单面扩张器与传统扩张器相比降低扩张时并发症另可多获得１５％～２０％的皮肤，以修复双面颊皮肤缺损。目前缺乏市售单面扩张器情况下，我们认为应用涤纶网硅胶板单面皮肤扩张器在颈部进行皮肤扩张，修复面颊部皮肤软组织是较为实用的方法。
The aim of this study was to expand the skin soft tissue by linear one-Way method which may get more“additional skin”grafts for skin coverage of the face in both sides and to reduce complications produced by the conventioal skin expander due to deep pressure. EPS with the possible volume of 250～300 ml was implanted in the deep layer of the platysma muscle. During management, according to the depth about 80 ml of water was injected in to the expander to bring an internal pressure and minimize local bleeding. The drainage tubes were also placed in it. Application of EPS in skin expansion operations in 32 patients showed a more effective decrease in complication than conventional skin expander. The former could expand “additional skin” by 15%～20% and could permit coverage in both sides of the face more than the latter. We think, it is wise to use the one-way expander with the sidlicon plate in expansion operation of neck skin.
