广西锡矿工人肺癌病例对照研究 被引量:5

A Nested Case-Control Study of Lung Cancer in Guangxi Tin Miners

摘要 选择广西四个锡矿1972~1974年队列共7855人进行队列内肺癌病例对照研究。追访到1994年底,发现肺癌死者132例(男130例,女2例排除),配对照627例。根据历年粉尘和已知致癌物的监测资料,定量计算所有对象的累积总粉尘接触量。经单因素和多因素非条件 Logistic回归分析,发现接尘与肺癌发病有关,调整吸烟后高接尘组肺癌危险度与中接尘组和低或无接尘组相比为1.8:1.6:1.0。进一步分析各锡矿肺癌发病特点显示,锡矿尘中致癌物砷含量高的大厂三个锡矿,接尘与肺癌关系明确(X2=8.27,P<0.05),而栗木锡矿未见接尘与肺癌相关(X2=2.74,P>0.05)。经比较两类锡矿,不支持游离二氧化硅致肺癌,粉尘似乎起着载体作用,矿尘中所含其他物质如砷等导致肺癌危险度升高。重度吸烟加重肺癌发病,与接尘相加作用不明显。 A nested case-control study of 130 male lung cancer cases and 627 controls wasselected from a cohort study of 7 855 subjects employed at least one year in 1974 to 1994 in fourGuangxi tin mines to investigate the risk of lung cancer by occupational factors and smoking habit.Cumulative total dust exposure was calculated for each individual based on historical industrial hygienerecords. Measurements on arsenic, asbestos polycyclic aromatic hydrocarbon (PAH) and radonwere also collected from the worksites. Ors, classical statistic analysis and non-conditional logisticregression with statistic analysis system (SAS) were used for analysis. The results showed that therisk of lung cancer was related to dust exposure and cigarette smoking. The Ors rates of high,medium and low exposure or no exposure were 1 .8: 1 .6: 1 .0 after adjust for smoking. Greatdifference was found when compared the risk of lung cancer in Dachang mine with Limn mine. Higharsenic concentration was monitored in Dachang, and a significant trend of increasing lung cancer riskwith cumulative dust exposure (X2 = 2. 74, P >0. 05 ) was observed in Limu mine with low arsenicconcentrations Based on these findings it could be concluded that dust acts as the carrier, thecoexisting components such as arsenic play an important role in carcinogenesis of tin miners' lungcancer.
