摘要
目的观察非生物型人工肝治疗重型肝炎后生化指标的改善与预后的关系。方法 383例重型肝炎患者在内科综合治疗的基础上进行不同类型的非生物型人工肝治疗,观察治疗前、治疗结束时及治疗后72 h谷丙转氨酶(ALT)、总胆红素(TB)、胆碱酯酶(CHE)、白蛋白(ALB)、凝血酶原时间(PT)及K、Na、C l的改善情况,以及PT、TB改善情况与预后的关系。结果人工肝治疗结束时与治疗前ALT、TB、K、PT均明显降低(P均<0.05),CHE升高(P<0.05),治疗后72 h ALT、TB、CHE、Na、K、PT均有不同程度的反弹,而人工肝治疗72 h后TB不反弹组、反弹但未超过治疗前组及反弹超过治疗前组近期疗效比较(P<0.05),差异均有统计学意义,人工肝治疗72 h后PT低于治疗前组近期疗效分别优于反弹但未超过治疗前组及超过治疗前组(P<0.05)。结论非生物型人工肝治疗后血清生化指标有显著改善,而第1次人工肝72 h后TB及PT反弹情况与预后密切相关,可据此对人工肝治疗疗效和预后进行评估。
Objective To observe the relationship between improved biochemical indicators and prognosis among severe hepatitis patients after artificial liver treatment.Methods A total of 383 patients with severe hepatitis were treated by different artificial liver treatments on the basis of internal medicine treatment.The relationship between biochemical indicators and prognosis among severe hepatitis patients were analyzed before treatment,at the end of treatment and at 72 hours after treatment.The biochemical indicators included alanine aminotransferase(ALT),total bilirubin(TB),cholinesterase(CHE),albumin(ALB),prothrombin time(PT) and K,Na,Cl.Results ALT,TB,K,PT were significantly lower(P<0.05),but CHE(P<0.05) was higher than those before treatment at the end of artificial liver treatment.ALT,TB,CHE,Na,K,PT had various degrees of rebound at 72 hour after artificial liver treatment.The short-term effects were compared among the group without TB rebound and the groups with lower and higher TB rebound than those before treatment at 72 hours after artificial liver treatment,the differences were statistically significant(P<0.05).The short-term effect in the group without PT rebound was better than those in the groups with lower and higher PT rebound than those before treatment at 72 hours after artificial liver treatment(P<0.05).Conclusion Serum biochemical indicators were improved significantly after artificial liver treatment.TB and PT rebound degree were closely related with the prognosis at 72 hours after the first artificial liver treatment,which could be used to evaluate treatment efficacy and prognosis.
