联合应用苯肾上腺素与麻黄碱维持剖宫产术中血压对产妇和新生儿的影响 被引量:3

摘要 目的比较同等效价的麻黄碱与苯肾上腺素用于产科麻醉维持产妇血压接近基础值时对产妇和新生儿影响的差异。方法 90例产妇拟在脊麻下实施择期剖宫产手术,随机分成三组泵注麻黄碱(Ⅰ组:4 g·L-1)、麻黄碱复合苯肾上腺素(Ⅱ组:麻黄碱2 g·L-1+苯肾上腺素25 mg·L-1)、苯肾上腺素(Ⅲ组:50 mg·L-1)维持血压接近基础值。术中测量注药后1、3、5、10 min、切皮时、子宫切开时的血压(MAP)、心率和胎心率;在分娩过程中分别取产妇动脉、脐动脉以及脐静脉的血液标本,检测血气分析、乳酸和血糖的浓度。结果Ⅰ组和Ⅱ组注药后胎心率明显增快(P<0.05),Ⅲ组胎心率无明显变化(P>0.05);Ⅰ组脐动脉和脐静脉血的pH值和碱剩余和低于Ⅱ组和Ⅲ组(P<0.05),Ⅰ组PCO2、乳酸和血糖浓度高于Ⅲ组(P<0.05)。结论联合应用苯肾上腺素与麻黄碱用于处理产科麻醉中的低血压有一定的优势。
