高危型HPV检测联合细胞学对宫颈病变的诊断价值 被引量:6

摘要 目的:评价单独和联合高危型人乳头状瘤病毒(HPV-DNA)检测和宫颈液基细胞学检查(TCT)对宫颈癌及癌前病变筛查的效果。方法:对885例妇女进行HPV分型基因检测技术检测高危型HPV和TCT检查,248例在电子阴道镜下进行可疑病灶定位活检,以宫颈活检的组织病理学结果为确诊标准。结果:经病理确诊为宫颈病变的患者,高危型HPV阳性率分别为CINⅠ45.00%、CINⅡ84.00%、CINⅢ93.00%、SCC100.00%,且HPV感染与CINⅡ/Ⅲ正相关(r=0.801,P<0.01)。HPV检测CINⅡ、Ⅲ的敏感度、特异度、阳性预测值、阴性预测值分别为90.12%、71.72%、5.13%、98.26%;宫颈细胞学筛查CINⅡ、Ⅲ,以LSIL为分界点的上述4值分别是89.35%、83.66%、21.52%、99.52%;高危型HPV检测联合细胞学检查的上述4值分别是98.72%、73.48%、51.52%、100.00%。结论:HPV高危型检测是有效的宫颈癌筛查方法,联合应用TCT后敏感度和阴性预测值进一步提高,但特异度未能提高。 Objective:To evaluate the effect of single and union high risk-human papillomavirus test(HPV)and liquid based cytology(TCT) in diagnosis of cervical disease.Methods:885 cases of patients were screened by combining HPV test(HPV Genotyping system)and TCT(liquid-based ThinPrep cytology test),248 cases were further biopsied under the colposcope and diagnosed with pathology.Results:Among the Patients with abnormal cervical lesions diagnosed with pathology,the positive rate of HPV in CINⅠwas 45%,CINⅡ84%,CINⅢ 93%,SCC100%,and were positively correlated with CINⅡ/Ⅲ(r=0.801,P〈0.01).High risk HPV DNA test the sitivity,specificity,positive forecasting value,negative forecasting value of CINⅡ,Ⅲ were separately 90.12 %,71.72 %,5.13%,98.26 %;TCT checking CINⅡand Ⅲ,the said four value division of LSIL were separately 89.35%,83.66%,21.52%,99.52%;and the said four values of combination of HPV DNA test union TCT checking were separately 98.72%、73.48%,51.52%,100%.Conclusion:High-risk HPV DNA test is a effective method for detecting cervical disease.The combination of high-risk HPV DNA test and cytological test increases the sensitivity and negative-predictive value,but it does not increase the specificity.
