盐酸法舒地尔对急性缺血性脑卒中患者FIB的影响 被引量:5

摘要 目的观察盐酸法舒地尔对急性缺血性脑卒中患者的纤维蛋白原(FIB)的影响及其与疾病转归的关系,探讨其治疗机制。方法选取住院确诊为急性缺血性脑卒中、并在48h之内接受治疗患者60例。将年龄、性别、病程及NIHSS评分相似的患者随机分为试验组和对照组,每组各30例。试验组为法舒地尔组,入院后立即给予盐酸法舒地尔注射液60mg/60min,静滴,2次/d(年龄超过70岁者,可选择30mg/30min,静滴,3次/d),疗程14d。对照组为血塞通组和巴曲酶组。血塞通组给予血塞通粉针400mg加入生理盐水250m|静脉滴注.1次/d,疗程14d。巴曲酶组首次给予10U加入生理盐水250ml静点,隔日给予5U加入生理盐水250ml静点,再隔日给予5U加入生理盐水250ml静点,每次用药前均检查FIB值。以上三组可同时酌情给予脱水剂、降压药、脑保护剂及对症治疗,均在治疗14d后复查FIB值。结果盐酸法舒地尔组治疗前后Rankin量表评分及NIHSS量表评分均有显著性差异;盐酸法舒地尔组对FIB的降低率高于血塞通组低于巴曲酶组。结论盐酸法舒地尔可以改善缺血性脑卒中患者的神经功能缺损症状,并降低血液中FIB值。盐酸法舒地尔对血液中FIB的降低率较血塞通强,较巴曲酶弱。由此可见,盐酸法舒地尔治疗缺血性脑血管病的治疗机制可能与降低血液中FIB值有关。
