摘要
目的探讨无创机械通气治疗慢性阻塞性肺病(COPD)合并意识障碍Ⅱ型呼吸衰竭的临床效果、可行性及安全性。方法对32例COPD并Ⅱ型呼吸衰竭伴意识障碍患者给予无创机械通气治疗,观察通气前、后2小时、结束时的神志变化及血气分析。结果 32例患者经NPPV(无创机械通气)治疗后23例好转出院,9例患者疗效不佳行气管插管。结论无创机械通气治疗慢性阻塞性肺病合并意识障碍Ⅱ型呼吸衰竭患者疗效肯定,有良好的安全性及可行性。
Objective To investigate the clinic effect, feasibility and security of the noninvasive positive pressure ventilation （NPPV） on the patient of chronic obstructive pulmonary disease （COPD） with consciousness disorders and type Ⅱ respiratory failure. Methods 32 patients of chronic obstructive pulmonary disease （COPD） with consciousness disorders and type Ⅱ respiratory failure were received the therapy of NPPV, and we observed the change of consciousness and blood gas analysis of the patients before the ventilation two hours after the ventilation and the end of the ventilation. Results 23 patients with NPPV improved and discharged from hospital. 9 patients were ineffective and received tracheal cannula. Conclusions For the patient of COPD with consciousness disorders and type Ⅱ respiratory failure, the NPPV had obvious effects, it had satisfactory feasibility and security.
