内窥镜辅助下微血管减压术治疗面肌痉挛 被引量:8

Endoscope-assisted Microvascular Decompression of the Hemifacial Spasm

摘要 目的:总结微血管减压术(MVD)治疗面肌痉挛的临床手术经验。方法:回顾性分析2004年12月至2010年2月经枕下乙状窦后入路面神经MVD(内窥镜辅助下显微手术)治疗面肌痉挛患者共258例。结果:手术有效256例(99.2%),其中治愈251例(97.3%),部分治愈5例(1.9%)。小脑前下动脉为相关责任血管占58.9%。术后并发症共28例(10.8%),其中,听力障碍13例,面瘫8例,耳鸣3例,复视1例,吞咽障碍1例,脑脊液漏2例。结论:MVD治疗面肌痉挛的疗效确切满意。内窥镜可显著提高手术的精确性,并减少手术并发症。 Aim:To summarize the clinical experience in the treatment of hemifacial spasm (HFS) with microvascular decompression (MVD). Methods:A retrospective study was performed by using the patient samples in 258 cases from 2004 to 2010. Suboccipital retrosegmoid approach was used in microsurgery and assisted by endoscope. Results:MVD was effective in 256 cases(99.2%) with no surgical death or major deficit. The excellent and good were 251(97.3%) and 5 (1.9%),respectively. The majority (58.9%) of affecting vessels was anterior interior cerebellar artery (AICA). Neurological complications were 28 cases(10.9%),which were hearing deficit(n=13),facial weakness(n=8),tinnitus(n=3),diplopia(n=1),deglutiton weakness(n=1),cerebrospinal leaking(n=2). Conclusion:MVD of the facial nerve is an effective and reliable treatment for HFS. Meanwhile,using of endoscopic vascular decompression is a safe and equally effective procedure,which can increase operative accuracy and decrease operative side-effect.
