4种含酶抑制剂复合药对常见非发酵菌体外抗菌活性的研究 被引量:1

In vitro antibacterial activity of complex preparation containing enzyme inhibitor to common nonfermenting gram-negative bacilli

摘要 目的了解含酶抑制剂复合药对常见非发酵菌体外抗菌活性,为临床抗菌药物的选用提供科学依据。方法对2008—2009年住院患者标本分离的566株非发酵菌,包括鲍氏不动杆菌(ABA)247株,铜绿假单胞菌(PAE)221株,嗜麦芽寡养单胞菌(SMA)82株,洋葱伯克霍尔德菌(BCE)49株,用K—B法检测4种含酶抑制剂复合药的体外药敏情况。结果4种非发酵菌对氨苄西林/舒巴坦均显示高度耐药,耐药率为85.0%~100.0%,对头孢哌酮/舒巴坦耐药率最低,为18.3%~34.7%;与原药比较,头孢哌酮/舒巴坦对ABA和SMA的敏感率提高最为明显,分别从27.1%和42.7%上升至78.5%和81.7%;哌拉西林/他唑巴坦对PAE和SMA也有较好的协同效果,敏感率分别从29.0%和17.1%上升至58.4%和52.4%。结论在选用含酶抑制剂复合药时必须根据该细菌的总体耐药状况、产生何种β-内酰胺酶以及原药的抗菌效果等综合考虑。 OBJECTIVE To study antibacterial activity in vitro of complex preparation containing enzyme inhibitor to common nonfermenting Gram-negative bacilli for the clinic choice of antibiotics. METHODS We collected 566 strains of Nonfermenting Gram-negative bacilli including 247 strains of Acinetobacter baumannii (ABA), 221 strains of Pseudomonas aeruginosa (PAE), 82 strains of Stenotrophornonas maltophilia (SMA) and 49 strains of Burkholderia cepacia (BCE) isolated from patients admitted to the hospital from 2008 to 2009 and tested the in vitro drug sensitivity to 4 kinds of Complex preparation containing enzyme inhibitor by K-B method. RESULTS Four kinds of Nonfermenting Gram-negative bacilli showed higher drug resistance to ampicillin/sulbactam (drug- resistance rate, 85. 0%-100. 0%) and lower drug-resistance to cefotaxime/sulbactam (drug-resistance rate, 18.3%-34.7%). Cefotaxime/sulbactam improved the sensitivity to ABA and SMA significantly as compared with the original drug which increased the sensitivity rate from 27.1% and 42.7 to 78.5% to 81.7%. Piperraeillin/ tazobactam showed great synergic effect to PAE and SMA which increased the sensitivity rate from 29.0K and 17. 1% to 58. 4% and 52. 4%. CONCLUSION The complex preparation containing enzyme inhibitor should be chosen depending on drug resistance status such as the kind of β-lactamase it produces and antibacterial effect of original drug, and so on.
