胸苷酸合成酶在乳腺癌中的表达及其与预后的关系 被引量:1

摘要 目的:研究乳腺癌组织中胸苷酸合成酶(thymidylate synthase,TS)的表达水平,评估TS作为5-氟尿嘧啶(5-FU)化疗敏感性的判断指标的可行性。方法:应用S-P免疫组化方法检测94例乳腺癌组织和40例乳腺非癌组织中TS的表达情况及其与临床各病理参数的关系。结果:①TS低表达率为55.3%(52/94),高表达率44.7%(42/94),与良性对照组比较有显著差异(P<0.01)。②随着乳腺癌病理组织学分级的程度增加,TS表达水平有明显增高趋势。Ⅰ级与Ⅲ级比较,差异有显著性(χ2=17.8756,P<0.01);TS表达与病人年龄、肿瘤大小、淋巴结转移、TNM分期的差异均无统计学意义(P>0.05)。③TS高表达组的5年生存率为26.5%,TS低表达组的5年生存率为58.7%。TS低表达组的生存期及5年生存率均高于TS高表达组,差异有统计学意义(P<0.05)。结论:乳腺癌组织中TS的表达与病人对5-FU化疗敏感性及预后密切相关,可作为判断5-FU化疗敏感性及预后的指标。 Objective To study the expression level of thymidylate synthase in breast cancer,and to assess the feasibility of TS as an indicator to determine the sensibility towards 5-fluorouracil(5-FU) chemotherapy.Methods Application of S-P immunohistochemical method in detecting the expression of TS and its relationship with clinical pathological parameters between the 94 cases of breast cancer and 40 cases of non-cancerous diseases of the breast.Results ① The low expression rate of TS was 55.3%(52/94),the high expression rate was 44.7%(42/94);the expression rate was compared with that of the healthy control group,and demonstrated significant difference(P0.01).② With the increase of histopathological grading of the breast cancer,the expression level of TS concomitantly increase significantly.There existed a significant difference between theⅠ and Ⅲ grade levels(χ2=17.8756,P0.01);there was no significant difference between the expression of TS and the age,tumor size,lymph node metastasis and TNM staging(P0.05).③ The 5-year survival rate of the group with high expression of the TS was 26.5%,the group with low expression of TS was 58.7%.The survival time and the 5-year survival rate of the low TS expression group was higher than that of the high expression group,and there existed a statistically significant difference between the 2 groups(P0.05).Conclusions The expression of TS in breast cancer patients is closely related to their sensibility towards 5-FU chemotherapy the expression of TS could be used as an indicator to determine the sensitivity towards 5-FU chemotherapy and the prognosis of the patients.
