摘要
目的了解2009年北京医院临床分离菌对常用抗菌药物的耐药性。方法采用纸片Kirby-Bauer法,对该院2009年全年分离的1 983株临床分离菌作药敏试验,采用WHONET5.4统计软件对数据进行分析。结果临床分离的细菌构成比中,前3位革兰阴性杆菌分别为铜绿假单胞菌467株(23.5%)、嗜麦芽窄食单胞菌285株(14.4%)和大肠埃希菌195株(9.8%);革兰阳性球菌中,前3位分别为金葡菌286株(14.4%)、肠球菌属68株(3.4%)和肺炎链球菌45株(2.3%)。体外药敏试验结果显示β溶血链球菌未见青霉素耐药菌株;流感嗜血杆菌产β内酰胺酶检测率为8.2%。金葡菌和凝固酶阴性葡萄球菌中耐甲氧西林株分别为78.3%(224/286)和65.3%(17/26)。发现5株耐万古霉素的屎肠球菌。大肠埃希菌超广谱β内酰胺酶检出率为53.3%,克雷伯菌菌属(肺炎克雷伯菌+产酸克雷伯菌)为34.2%,铜绿假单胞菌和不动杆菌属中已分别出现了2.6%(12/467)和21.4%(27/126)的泛耐药菌株。结论定期进行耐药性监测有助于了解该院细菌耐药性变迁,为临床经验用药提供理论依据。
Objective To investigate the resistance of clinical isolates from Beijing Hospital in 2009. Methods Kirby-Bauer method was used to test the susceptibility of 1983 clinical isolates to antimicrobial agents. The data were analyzed by WHONET software. Results The top 3 gram negative pathogens were Pseudomonas aeruginosa （467, 23.5%）, Stenotrophomonas maltophilia （285, 14.4%） and Escherichia coli （195, 9. 8%）. The top 3 gram positive cocci were Staphylococcus aureus （286, 14.4%）, Enterococcus spp. （68, 3.4%） and Streptococcus pneumoniae （45, 2.3%）. The susceptibility testing results showed that no penicillin-resistant 13-hemolytic Streptococcus was found. Beta-lactamase was produced by 8.2%o of the Haemophilus influenzae strains. The prevalence of rnethicillin-resistant Staphylococcus was 78.3% （224/286） in S. aureus and 65.3% （17/26） in coagulase-negative Staphylococcus. Five strains of Enterococcus faecium were resistant to vancomycin. ESBLs-producing strains accounted for 53.3 % in E. coli and 34.2% in Klebsiella spp. Multi-drug resistant strains were iden tiffed in 2.6 %（12/467） of P. aeruginosa isolates and 21.4 % （27/126） of Acinetobacter spp. Conclusions The data of periodic bacterial resistance surveillance are most important and valuable for rational selection of antimicrobial agents.
