摘要
目的通过探讨上颌窦形态参数与阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者病情程度的关系,加深对上气道阻塞成因的认识,进一步明确鼻腔对气道阻塞的影响。方法 对79例经多导睡眠监测(polysomnography,PSG)确诊的OSAHS患者行上气道CT扫描,通过图像分割三维重建上颌窦,测量与上颌窦形态相关的参数,并与呼吸暂停和低通气指数(apnea and hypopnea index,AHI)、最低动脉血氧饱和度(lowest oxygen saturation,LSaO2)进行相关性分析。结果 代表上颌窦形态整体指标的上颌窦体积与AHI呈正相关(r=0.595,P<0.05)。在其他相关参数中气化系数、X轴长度是相关性最强的两个。上颌窦两侧不对称也对病情程度产生了一定的影响。上颌窦形态参数与LSaO2大都没有相关性。结论 OSAHS患者的上颌窦形态对其病情程度有一定的影响。上颌窦的气化过程在与周围结构的相互作用中,与上气道硬性部分尤其是鼻腔相互影响,从而造成上气道通气功能障碍。
Objective:To investigate the correlation between the measurement of 3-D maxillary sinus and the severity of patient with obstructive sleep apnea hypopnea syndrome.Methods:Seventy-nine patients with obstructive sleep apnea-hypopnea syndrome(OSAHS) received CT scan.Then the images of maxillary sinuses were segmented and reconstructed in order to form 3-D image.Correlation analyses were made between the measurements of the 3-D maxillary sinuses and apnea and hypopnea index(AHI),the lowest oxygen saturation(LSaO2).Results:The parameters of the maxillary sinus positively correlated with AHI,particularly the volume,coefficient of pneumatolysis,the length of the X axis.The asymmetry of the development of maxillary sinuses did influence the severity of OSAHS.No correlation was found between LSaO2 and the shape of maxillary sinus.Conclusion:The development of maxillary sinus affects the structure of the facial cranium,in particular the nasal cavity;the facial cranium,in turn,may have an influence on the dimensions of maxillary sinus.The correlation between severity of OSAHS and the dimension of maxillary sinuses suggest that upper airway obstruction may result from abnormalities of bony canal,especially the nasal cavity.
