摘要
目的了解和探索胸外科手术患者围术期的真实感受。方法采用目的性抽样,选取长海医院胸心外科住院患者共10例,采用半结构式访谈提纲,深度访谈并记录10例胸心外科术后患者的感受。结果访谈共提炼出6个主题:(1)关注患者的心理状态至关重要;(2)ICU期间呼吸机使用的痛苦是麻醉苏醒时的第一不适;(3)确定手术效果是患者麻醉苏醒后的第一需要;(4)睡眠障碍是ICU期间较突出的问题;(5)患者ICU治疗期间心理负担过重;(6)文化水平与患者需求之间具有相关性。结论胸心外科围术期患者生理和心理都受到极大的冲击,护理人员应提供系统完善的整体护理,根据患者文化程度和认知水平的不同,为其提供适宜的护理措施,系统性和有针对性地提供临床护理,以减轻患者的多种不适。
Objective To explore the real experience of patients in cardiothoracic surgery department during the perioperative period.Methods By purposive sampling,10 patients were chosen in Department of Cardiothoracic Surgery,Changhai Hospital.Semi-constructed interviews were performed among the 10 patients about their real experience during the perioperative period.Results Six themes were refined：（1） It was necessary to pay attention to patients＇ negative psychological state;（2）Ventilator-related discomfort was patients＇ first concern in ICU after anesthesia recovery;（3） It was the first requirement to inform patients of the surgery results.（4）Sleeping disturbance was an serious subject to deal with for patients in ICU;（5） Patients suffered from serious psychological burdon in ICU;（6） Education level was correlated with the needs of patients.Conclusion Patients in cardiothoracic surgery department suffer from dramatic attack both physically and psychologically during the perioperative period.According to patients＇ education level and cognition level,nursing staff should supply systemic and corresponding nursing service to patients to alleviate patients＇ discomfort.
