摘要
目的探讨妊娠中期糖代谢异常孕妇胰岛自身抗体的分布情况及其阳性对糖尿病临床特征的影响。方法选择97例妊娠期糖尿病(GDM)与妊娠期糖耐量受损(GIGT)孕妇作为研究对象,测其孕24~28周的胰岛自身抗体,包括谷氨酸脱羧酶自身抗体(GADA)、胰岛细胞自身抗体(ICA)及胰岛素自身抗体(IAA),同时对胰岛自身抗体阳性组和阴性组孕妇的临床特征进行分析。结果 97例糖代谢异常孕妇的GADA、ICA及IAA的阳性率分别为8.2%、4.1%、23.7%,IAA的阳性率显著高于GADA以及ICA(P<0.01);35.0%的糖代谢异常孕妇至少一项胰岛自身抗体为阳性;3种胰岛自身抗体在GDM及GIGT孕妇中的阳性率差异无统计学意义;GADA阳性组的OGTT 2 h血糖比阴性组低(P<0.01);自身抗体阳性组与阴性组孕妇的临床特征差异无统计学意义。结论约1/3妊娠期糖代谢异常孕妇具有胰岛自身抗体,以IAA的阳性率最高;GDM与GIGT具有相似的胰岛自身免疫特点;胰岛自身抗体阳性组及阴性组糖代谢异常孕妇在孕中期的临床特征相似。
Objective To evaluate the spectrum of islet autoantibodies in women with gestational hyperglycemia for investigation of its significance in gestational hyperglycemia.Methods Ninety-seven women with gestational diabetes mellitus(GDM) or GIGT gestational impaired glucose tolerance(GIGT) were enrolled.Islet autoantibodies,including glutamic acid decarboxylase autoantibodies(GADA),islet cell autoantibodies(ICA) and insulin antoantibodies(IAA),were detected in at the 24th to 28th weeks of pregnancy.Results GADA,ICA and IAA positive rates were 8.2%,4.1% and 23.7%,respectively,with significant difference revealed between IAA and GADA or ICA(P0.01).Positive islet autoantibody was found in 35.0% of subjects,with no significant difference revealed between those in GDM and GIGT groups.The 2-h blood glucose of OGTT in autoantibody positive group was significant lower than that in autoantibody negative group(P0.01).There was no significant difference in clinical characteristics between autoantibody positive group and autoantibody negative group.Conclusion Positive islet autoantibody is presented in about 1/3 women with gestational hyperglycemia,and IAA accounted for the majority.GDM and GIGT women share similar islet autoimmune response.Positive autoantibody complicates no specific clinical characteristics in gravidae during the second trimester.
