血浆TGF-β_1和MMP-9在新生儿不同氧疗方式中的变化

摘要 目的探讨氧疗新生儿血浆中转化生长因子-β1(TGF-β1)和基质金属蛋白酶-9(MMP-9)的变化及临床意义。方法经氧疗的新生患儿68例,其中经鼻导管氧疗31例,头罩氧疗25例,呼吸机氧疗12例,采用ELISA方法测定氧疗前,氧疗第2天及治愈后血浆中TGF-β1和MMP-9的水平;对照组为同期婴儿室20名未经过氧疗的正常新生儿。结果与正常对照组相比,鼻导管氧疗患儿血浆中TGF-β1和MMP-9的水平增高,但不具有统计学差异,头罩氧疗组血浆TGF-β1和MMP-9的水平明显增高,而给予呼吸机氧疗的患儿增高最为明显,相关分析表明,MMP-9与TGF-β1呈正相关,与氧疗的时间和氧浓度具有正相关关系。结论血浆MMP-9与TGF-β1在面罩和呼吸机氧疗新生儿中明显增高,与氧疗的时间和浓度具有正相关关系,氧疗过程中具有一过性的肺损伤,监测血浆中TGF-β1和MMP-9或许对指导氧疗具有一定的临床意义。 Objective To investigate changes and clinical significance of plasma transforming growth factor-β1(TGF-β1) and matrix metalloproteinase-9(MMP-9) in neonatal with different oxygen therapy approach.Methods 68 neonatal oxygen therapy cases were studied,31 cases of children with nasal oxygen catheter,25 cases with oxygen mask and 12 cases of children with ventilator oxygen therapy.Plasma TGF-β1 and MMP-9 level was determined by ELISA before oxygen therapy,oxygen therapy after 2 days and after cure.The same period 20 normal infants have not been oxygen therapy as control.Results Compared with normal control group,children with nasal catheter oxygen plasma TGF-β1 and MMP-9 levels increased,but not statistically different.Plasma TGF-β1 and MMP-9 levels were significantly increased in infant with oxygen mask therapy,but the infant given oxygen therapy with ventilator increased most obviously.Correlation analysis showed TGF-β1 and MMP-9 was positively correlated with the time and oxygen therapy concentration.Conclusion Plasma TGF-β1 and MMP-9 in the infant with mask and breathing machine oxygen therapy was significantly higher in neonates,and had a positive correlation with the time and the concentration of oxygen therapy.A transient lung injury was occurred during oxygen therapy.Monitoring plasma TGF-β1 and MMP-9 had some clinical significance to guide oxygen therapy.
