摘要
目的为了有效缓解居民"看病难、看病贵"的重要问题,郑州市提出并实践了"片医负责制"社区卫生服务体系。本文对郑州市"片医负责制"在社区卫生服务体系建设中的实践情况进行了总结,以期为其他地区的社区卫生工作提供借鉴和启发。方法 "片医负责制"是社区卫生服务机构的医护人员以"团队"形式上门服务,建立家庭健康档案,以政府规定的预防保健任务为主,从传染病管理、妇女儿童和老年人保健、慢性病管理及残疾人康复等方面,向社区家庭的全体成员提供全程保健服务的新模式。结果目前,郑州中心城区、高新技术开发区、经济技术开发区、郑东新区共有188家社区卫生服务机构,以"片医负责制"为特色的城市社区卫生服务覆盖面达到280万人,基本形成了多区域、深层次、广覆盖的城市社区卫生服务网络,形成了"小病在社区、大病进医院、康复回社区"的就医模式,初步实现了基本公共卫生服务均等化。结论 "片医负责制"使每个片医小组都有固定的责任服务区,每户居民也都有固定的责任医护人员,社区卫生服务增强了针对性和目标性,可以最大范围地掌握辖区居民的健康资料,既提高了社区医生对居民健康的管理能力,也加深了居民对社区卫生服务工作的理解和信任,为有效缓解群众就医难题和推进基本公共卫生服务均等化探索出一条可行的有效途径。
Objective In order to tackle the problems of difficulties in seeing a doctor and high medical costs,Zhengzhou Health Bureau had set up ＂District Doctors′ Responsibility System＂ in community health centre.In this paper we summed up the practical conditions of the system in development of community health services system in Zhengzhou so as to provide a reference or elicitation for community health services in other areas.Methods ＂District Doctors′ Responsibility System＂ is a new model in which medical professionals at community health centre as a team provide overall health care services for all family members of residents by setting up family health records to prevent illness and promote health well-being,which are tasks set by Chinese government.The services include control of infectious diseases,health care for women,children and elderly people,management of chronic diseases,and rehabilitation for the disabled.Results At present there are 188 community health service agencies in Zhengzhou including the area of central town,innovative and high technology development zone,economic-technology development zone,and Zhengdong new district.Urban community health services characterized by ＂District doctor′s Responsibility System＂ cover area of 2 800 000 people.An urban community health service network of multi-regional,deep and wide coverage has been basically set up.A pattern of seeing a doctor has taken shape,i.e.,＂a minor illness is treated in the community,a serious disease is treated in the hospital,and people needing rehabilitation return to the community＂,and thus equalization of primary public health service has been realized preliminarily.Conclusion ＂District doctors′ Responsibility System＂ makes each team of doctors and nurses have their own service area to take responsibilities.Every resident has his or her own fixed medical professional.Community health service strengthens its pertinence and targetedness,and makes it possible to know relevant data regarding local residents′ health conditions at maximum level.For community doctors,the service has improved their management about residents′ health.On the other hand,it helps to deepen the understanding of the community health service by local people and their and trust in it.＂District doctors′ Responsibility System＂ could be a feasible and effective way to solve current difficulties in seeing a doctor and to carry forward the equalization of public health services.
