摘要
目的了解巢湖市新发肺结核病人就诊延迟情况,通过分析病人的社会经济学特征及影响就诊延迟的因素,确定就诊延迟的高危人群,为早期发现涂阳肺结核病人,采取有效的预防措施提供依据。方法使用专门设计的结构式问卷对新发现的539例涂阳肺结核病人进行调查,采用定量研究的方法对病人的就诊延迟进行单因素分析。结果 539例新发现的涂阳肺结核病人中,有243例就诊延迟,延迟率为45.1%;不同性别、年龄、文化程度之间延迟率的差异有统计学意义,不同职业之间延迟率的差异无统计学意义。女性、老年人、文盲是就诊延迟的高危人群。病人经济困难、缺乏对结核病的了解、防范意识淡薄、怕被别人歧视、工作忙以及交通不便是导致病人就诊延迟的主要原因。结论涂阳肺结核病人就诊延迟率较高,应明确就诊延迟的高危人群并针对影响就诊延迟的因素开展积极有效的预防干预措施。
Objective To understand the delay in seeking health of new smear-positive tuberculosis patients in Chaohu City and analyze socioeconomic characteristics of the patients and other factors on the impact of the delay in seeking health of patients,identify the high-risk groups of delay,provide evidence for detecting smear-positive TB patients early and taking effective preventive measures.Methods The specially designed questionnaire was used to study 539 new smear-positive TB patients in an area of Anhui,and quantitative research method was used to investigate the delay in seeking health of patients,all data were analyzed using single-factor analysis.Results Of 539 newly discovered pulmonary tuberculosis patients,243 patients had delay,the delay rate was 45.1%;there was significant difference in delay rate between different gender,age and education level,while no significant difference was found in delay rate between different profession.Women, elderly,illiteracy were high risk groups of delay.Patients with financial difficulties,lack of knowledge of tuberculosis,weak awareness of prevention,fear of being discriminated,busy with work and inconvenience of traffic were the main reasons leading to patients delay.Conclusions Delay rate of smear positive tuberculosis patients was relatively higher,the high risk of delay and high-risk groups of delay should be identified clearly,for factors that affect delay,active and effective preventive interventions should be carried out.
