摘要
目的探讨正常犬在肝硬化形成过程中胃酸分泌功能、胃黏膜保护性激素和胃组织病理学变化及其相互关系。方法选用杂种犬16只,以40%四氯化碳(CCl4)腹腔内注射建立犬肝硬化动物模型,用Digtrapper MKⅢpH监测仪监测正常犬和肝硬化犬24 h胃内pH值变化,同时用放射免疫法检测在造模前后血浆生长抑素(SS)和前列腺素E2(PGE2)水平的变化。经解剖组织切片观察肝硬化犬胃组织病理学改变。结果肝硬化犬胃内pH值比正常犬显著升高(P<0.01),胃酸度降低;血浆SS和PGE2含量明显下降(P<0.01);病理组织学显示肝硬化犬胃窦、体黏膜有炎细胞浸润,黏膜肌及黏膜下纤维组织增生,腺体数目减少或腺体萎缩,壁细胞数目减少,黏膜及黏膜下血管扩张淤血。结论肝硬化形成伴门静脉系淤血缺氧,胃黏膜保护性激素水平下降,胃壁细胞数减少,胃酸分泌减少,是造成门静脉高压性胃病和肝病相关性溃疡的病理基础,与消化性溃疡发病机制完全不同。
Objective To observe the changes of gastric acid secretion,hormonal of protecting gastric mucous,and histopathology when hepatic cirrhosis is being formulated in normal dogs.Methods 16 crossbred dogs were selected and rendered to hepatic cirrhosis model with intraperitoneal injection of 40% CCl4.The changes of 24-hour intragastric pH in normal dogs and hepatic cirrhosis dogs was monitored by Digtrapper MKⅢ,and the level of plasma SS and PGE2 was determined by radioimmunodetection before and after model building.The pathological changes of stomach issues in hepatic cirrhosis dogs were observed by pathology.Results When compared with that in the normal dogs,the intrgastric pH in hepatic cirrhosis dogs was significantly higher,and gastric acidity was lower(P0.01).Both SS and PGE2 levels in the hepatic cirrhosis dogs were significantly lower than that in the normal dogs(P0.01).Pathological results of hepatic cirrhosis dogs showed inflamed cells imbibitions in mucous membrane of gastric antrum and corpus,fibroplasia in mucous membrane muscles and submucosa,glandular organ reduction or atrophy,acid cell reduction,angiectasis and congestion in mucous membrane muscles and submucosa.Conclusion When hepatic cirrhosis formulates,portal vein congestion and anoxia,lower level of protecting hormonal in gastric mucosa,reduction of acid cell and gastric acid secretion are the pathological basis of hypertension portal gastric disease and ulcer related to liver disease,which differs completely from peptic ulcer.
