摘要
目的:了解中山大学附属东华医院2008年临床常见病原菌对常用抗菌药物的耐药情况,为临床合理用药提供参考依据。方法:纸片扩散法(Kirby-Bauer)检测临床分离菌对各种抗菌药物的耐药性,参照2008版CLSI标准判定药敏结果,并用WHONET5.4软件进行统计分析。结果:全年共检出细菌3 594株,其中革兰阳性菌占39.87%,革兰阴性菌占49.67%。前5位的细菌依次为:大肠杆菌、肺炎克雷伯菌、铜绿假单胞菌、金黄色葡萄球菌和凝固酶阴性葡萄球菌。对抗生素耐药性检测结果显示,MRSA检出率为26%,MRCNS检出率为80%;未发现耐万古霉素的葡萄球菌;粪肠球菌和屎肠球菌对高浓度的庆大霉素的耐药率分别为46.2%和48.1%,1株粪肠球菌对万古霉素不敏感,各种肠道杆菌均对亚胺培南敏感;铜绿假单胞菌耐亚胺培南的菌株占15.4%。结论:我院住院患者分离的细菌耐药性比较严重,对多数常用抗菌药物耐药率呈上升趋势。
OBJECTIVE: To investigate the resistance of the common clinical pathogens to commonly used antibiotics in our hospital in 2008 for references of clinical rational drug use.METHODS: Antimicrobial susceptibility test was carried out by Kirby-Bauer method,and the results of the drug susceptibility test were assessed in accordance to CLSI(version 2008) criteria wiht WHONET 5.4 software used for statistical analysis.RESULTS: A total of 3 549 strains of pathogens were isolated from various clinical specimens in 2008,with gram positive cocci and gram negative bacilli accounting for 39.87% and 49.67%,respectively,leading the top 5 places were E.coli,K.pneumoniae,P.aeruginosa,S.aureus and coagulase negative staphylococcus.The antimicrobial resistance test showed a detection rate of 26.0% for MRSA(methicillin resistant staphylococcus aureus) and 80% for MRCNS(methicillin-resistant coagulase negative Staphylococcus).No vancomycin-resistant staphylococcus strain was noted.The resistance rates of E.faecalis and E.faecium to high concentration of gentamicin(120 μg) were 46.2% and 48.1%,respectively.One E.faecalis strain was noted to be insensitive to vancomycin.Isolates of Enterobacteriaceae were all susceptible to imipenem.15.4% of P.aeruginosa strains were resistant to imipenem.CONCLUSION: The bacterial drug resistance in the clinical isolates from our hospital is serious with increasing resistance to most antimicrobial agents.
