摘要
目的探讨血清铁蛋白(SF)与肥胖、初发2型糖尿病的相关性,并探讨SF增高是否与胰岛素抵抗具有关联性。方法按体重指数(BMI)≥25kg/m2将73例口服葡萄糖耐量试验正常者和86例初发2型糖尿病患者(T2DM)分为正常糖调节/正常体重组、正常糖调节超重/肥胖组、T2DM正常体重组、T2DM超重/肥胖组等4个亚组。测定所有研究对象的SF、空腹胰岛素、空腹血糖、糖化血红蛋白、血脂、尿酸及血压水平,并计算BMI,腰臀围比及胰岛素抵抗指数。结果肥胖、初发2型糖尿病具有较高的SF水平,与正常糖调节/正常体重组相比,有显著性差异(P<0.05);SF与BMI、腰臀围比、收缩压,空腹胰岛素、空腹血糖,胰岛素抵抗指数、甘油三酯及尿酸呈正相关,且SF与胰岛素抵抗指数存在一定剂量关系。结论肥胖、初发T2DM患者体内铁超负荷,而铁超负荷可能导致胰岛素抵抗。
Objective To study the correlation between serum ferritin(SF) and newly diagnosed type 2 diabetes mellitus with obesi- ty,and to evaluate the relevance of increased serum ferritin and insulin resistance. Methods According to BMI ≥25kg/m^2,73 patients with normal oral glucose tolerance test and 86 newly diagnosed type 2 diabetes mellitus (T2DM) were divided into four subgroups.The SF,fasting insulin,fasting blood glucose, glycosylated hemoglobin,serum lipids,serum uric acid and blood pressure were measured and BMI,waist-hip ratio and insulin resistance index were calculated respectively. Results The newly diagnosed type 2 diabetes mellitus with obesity had higher serum ferritin levels than the normal glycoregulation normal weight group. There was a positive correlation between SF and BMI,waist-hip ratio,systolic blood pressure,fasting insulin,fasting blood glucose,insulin resistance index,serum triglyceride and serum uric acid levels respectively.Otherwise,there was a certain dose relationship of the SF with insulin resistance index. Conclusion This study showed that the patients of newly diagnosed type 2 diabetes mellitus obesity,had iron overload of the body, while iron overload may lead to insulin resistance.
