摘要
目的探索多囊卵巢综合征(PCOS)者多项代谢指标及其与性激素的关系。方法测定并比较33例PCOS者和56例初诊2型糖尿病者体重指数、血压、血脂、尿酸、血糖、胰岛素和HOMA-IR等多项代谢指标及性激素水平。比较不同糖代谢水平的PCOS者行GnRH垂体兴奋试验后LH的变化曲线,并采用Pearson相关性分析研究LH变化和各项代谢指标的关系。结果 PCOS者的多项代谢指标(包括体重指数、尿酸和HOMA-IR)水平均明显高于2型糖尿病者(P<0.01或P<0.05)。注射GnRH后,PCOS合并血糖异常者的LH高峰值更明显,ΔLH和胰岛素、HOMA-IR呈正相关。结论 PCOS较2型糖尿病有更加严重的代谢紊乱和胰岛素抵抗,可影响下丘脑-垂体-性腺轴功能。
Objective To investigate the metabolic parameters of polycystic ovarian syndrome(PCOS) and to evaluate the correlation between the metabolic parameters and sex hormones.Methods Thirty-three cases of PCOS and 56 cases of newly-diagnosed type 2 diabetes mellitus were selected in this study.The metabolic parameters including body mass index(BMI),blood pressure(BP),blood lipid,uric acid(UA),plasma glucose,insulin and HOMA-IR index,together with the sex hormones were measured and compared.The luteinizing hormone(LH) variation curves after GnRH stimulation test of pituitary gonadotrophin were compared among different glucose tolerance groups in PCOS.The correlation between LH and metabolic parameters was studied by Pearson correlation analysis.Results The levels of metabolic parameters including BMI,UA and HOMA-IR index in PCOS group were higher than these in type 2 diabetes group(P0.01 or P0.05).After GnRH injection,the peaks of LH were higher in the PCOS groups with abnormal glucose tolerance.The correlation analysis showed that ΔLH was positively correlated with insulin and HOMA-IR index.Conclusion PCOS patients have more severe metabolic disorders and insulin resistance than type 2 diabetes patients.It will affect the function of hypothalamus-pituitary-gonad axis.
