摘要
目的:观察复方苦参注射液辅助治疗多发性骨髓瘤(MM)患者骨痛的临床效果。方法:88例MM患者给予常规诱导和巩固化疗同时,随机分为治疗组(45例)和对照组(43例)。治疗组接受复方苦参注射液联合帕米磷酸二钠治疗,对照组仅接受帕米磷酸二钠。给予复方苦参注射液15～25mL·d-1,每月15d;帕米磷酸二钠90mg·d-1,每月1次;均静脉滴注,qd,至少治疗4个月并维持治疗12个月以上,评价MM患者骨痛缓解情况。结果:治疗组和对照组化疗有效率分别为57.8%和53.5%。治疗组伴骨痛患者在第4、9个疗程后骨痛缓解有效率分别为90.0%和89.7%,对照组分别为62.9%和73.9%;2组化疗有效者和轻度骨痛者第4个疗程后骨痛均可达中度和完全缓解,2组化疗无效者第4、9个疗程骨痛缓解有效率分别为78.6%和33.3%、76.9%和70.0%;2组中重度骨痛者第4、9个疗程后骨痛缓解有效率分别为83.3%和33.3%、93.3%和83.3%。结论:复方苦参注射液早期辅助治疗MM可明显减轻患者骨痛,不良反应少。
OBJECTIVE： To observe the clinical efficacy of compound Sophora flavescens for bone pain of multiple myeloma patients（MM）. METHODS： 88 initial MM patients were received induce and consider chemical therapy, randomly assigned to treatment group（45 cases） and control group（43 cases）. Treatment group received compound S. flavescens injection and pamidronate, control group received pamidronate only. Compound S. flavescens injection（15～25 mL·d-1,iv.gtt.）, 15 d in per month. Pamidronate was given 90 mg·d-1 per month,iv.gtt. the adjuvant and maintain therapy must receive 4 months and 12 months respectively. The clinical efficacy was compared between 2 groups. RESULTS： The chemical therapy efficacy of treatment and control group was 57.8% and 53.5%. The clinical response rate of bone pain patients in treatment group after 4 and 9 cycles was 90.0% and 89.7%, the clinical response rate of bone pain patients in control group after 4 and 9 cycles was 62.9% and 73.9%. Bone pain patients of light degree pain or hematology remission after 4 cycles chemical therapy could reached moderate and complete degree pain remission, the clinical response rate of non-hematology response patients after 4 and 9 cycles chemical therapy was 78.6% and 33.3%, 76.9% and 70.0% respectively, the clinical response rate of moderate and major degree bone pain patients after 4 and 9 cycles chemical therapy was 83.3% and 33.3%, 93.3% and 83.3% respectively. CONCLUSION： Compound S. flavescens injection could alleviate bone pain of MM patients as an adjunctive therapy, and has low adverse reaction.
