摘要
目的探讨血尿酸对冠心病患者血管内皮功能的影响,寻找血尿酸作为冠心病危险因素的理论基础。方法以2009年8月—2010年8月因"胸闷、胸痛"于温岭市第一人民医院住院、并接受冠状动脉造影的患者为研究对象,分为冠心病组141例和对照组123例。检测两组患者的血尿酸水平,同时利用高分辨率超声观测肱动脉扩张力判断血管内皮功能。比较两组患者血尿酸和血管内皮功能相关指标的差异,并进行相关和回归分析,探讨血尿酸对血管内皮功能有无影响。结果 (1)两组间性别、年龄、吸烟史、高血压病史、收缩压、舒张压、体质指数、血糖、三酰甘油、总胆固醇、低密度脂蛋白及高密度脂蛋白等指标间差异均无统计学意义(P>0.05)。(2)冠心病组血尿酸水平(396.64±64.97)μmol/L显著高于对照组(334.67±56.46)μmol/L,差异有统计学意义(P<0.05)。冠心病组肱动脉血流介导的血管舒张功能(FMD%)(7.14±0.67)与对照组(13.56±1.27)比较,差异有统计学意义(P<0.05);硝酸甘油介导的血管舒张功能(FMN%)在两组间水平相当,差异无统计学意义(P>0.05)。(3)冠心病组FMD%与血尿酸水平呈直线负相关(r=-0.219,P=0.009);以FMD%为应变量(Y),以血尿酸为自变量(X)进行直线回归分析,直线回归方程为Y=8.036-0.002X。结论冠心病组患者与对照组比较血尿酸水平升高,血管内皮功能降低。血尿酸可能通过损伤血管内皮功能促进冠心病的发生和发展。
Objective To investigate the influence of the level of serum uric acid(SUC) on vascular endothelial dysfunction in patients with coronary heart disease(CHD) and thus to fined the theoretical basis of SUC as the risk factor of CHD.Methods All the patients came from The First People′s Hospital of Wenling,who were hospitalized because of pectoralgia or/and chest distress from August 2009 to Augst 2010,and all received coronary angiography.Among them 141 were in CHD group and 123 in control group.All the patients were given measurement of SUC and assay of the brachial artery tension by using High Resolution Sonography.The results were compared,and linear correlation and regression analysis were also conducted so as to study the influence of serum uric acid on the parameters of vascular endothelial function.Results(l)There were no significant difference between the two groups in parameters of sex,age,history of smoking,history of hypertension,systolic and diastolic blood pressures,body mass index,blood sugar,triglyceride,total cholesterol,lower density lipoprotein-cholesterol,and high density lipoprotein-cholesterol(P0.05).(2)The SUC level(396.64±64.97)μmol/L of the CHD group was significantly higher than that(334.67±56.46)μmol/L of the control group(P0.05).The level of serum uric acid in CHD group increased significantly than that in control group.The flow-mediated dilation(FMD) of the brachial artery in the CHD group(7.14±0.67),but no significant difference in nitroglycerin-induced dilation(NID) of the brachial artery was found(P0.05).(3)FMD in the CHD group was negatively correlated with SUC level(r=-0.219,P=0.009).A linear regression equation obtained by linear regression analysis was Y=8.036-0.002X as FMD was taken as the dependent variable(Y) and SUC as the independent variable(X).Conclusion The patients with CHD have the increased serum uric acid level,and vascular endothelial dysfunction.The serum uric acid may promote the occurrence and development of CHD through injuring the vascular endothelial function.
