强脉冲光治疗鲜红斑痣的临床及模拟研究 被引量:7

Treatment of Port Wine Stains with Intense Pulsed Light:a Clinical and Experimental Study

摘要 目的从临床治疗和理论分析两方面评价强脉冲光系统是否适用于治疗面、颈部鲜红斑痣。方法用Lumenis ONE强脉冲光治疗鲜红斑痣患者35例,用蒙特卡罗模拟方法对该强光系统的光子与脉冲染料激光两个波长的光子在皮肤组织中的传输过程及能量沉积进行模拟比较。结果强脉冲光治疗鲜红斑痣临床疗效显著,82.1%的患者达到>50%的清除率,其中28.6%的患者达到>75%的清除率,与国外文献报道的脉冲染料激光治疗鲜红斑痣的疗效十分接近。模拟结果显示,强脉冲光系统和595 nm激光系统的光子在血管中的光能量分布比577 nm激光系统的光子更为均匀,穿透深度更大。结论强脉冲光系统可以成为临床治疗面、颈部鲜红斑痣的良好选择。 Objective To evaluate theoretically and clinically whether intense pulsed light system is suitable for treating facial and neck port wine stains. Methods In this work,the latest Lumenis ONE intense pulsed light system was used to treat facial and neck port wine stains in 35 cases.The Monte Carlo method was used to imitate the treatment of intense pulsed light system and two kinds of pulsed dye lasers. Results It showed significant clinical effect.There were 82.1% of patients achieving more than 50% efficiency,of which 28.57% of patients could achieve more than 75% efficiency,which were in agreement with the pulsed dye light treatment from foreign literature.Theoretically the 577 nm was compared with 595 nm pulsed dye laser and intense pulsed light,and the later two distribution of light energy was more even and deeper. Conclusions Therefore,the intense pulsed light system can be a good choice for clinical treatment of facial and neck port wine stains.
