摘要
目的:研究头孢曲松钠联合青霉素治疗病原菌未明的化脓性脑膜炎(简称化脑)疗效。方法:治疗组:头孢曲松钠100 mg/(kg.d),分2次静脉输注,青霉素60～80万u/(kg.d)分3～4次静脉输注,辅助治疗地塞米松0.15 mg/(kg.d)分4次静脉输注,连用4 d.对照组:头孢曲松钠?青霉素及地塞米松剂量用法同治疗组另加用氯霉素75 mg/(kg.d),1次/d静脉输注。治疗组、对照组疗程均为3周。结果:治疗组平均体温正常时间2.75 d,对照组3.75 d,t=1.56,P>0.05;治疗组2周脑脊液正常16例(80.0%),对照组为15例(75.0%),χ2=0.649,P>0.05,治疗组并发症1例(5.0%),对照组2例(10.0%),χ2=0.358,P>0.05。结论:头孢曲松钠联合青霉素治疗病原菌未明的化脓性脑膜炎疗效确切,无需加用氯霉素,且价格适中,可推广使用。
Objective To investigate the effect of purulent meningitis with undetermined pathogenic bacteria treated by ceftriaxone sodium combined with penicillin G.Method Forty patients were allocated into two groups randomly.In the treatment group,the administration plan was celtriaxone sodium 100 mg/（kg·d）,bid,intravenous drip,penicillin G 600 000～800 000 unit/（kg·d）,3～4 times/d,intravenous drip,and dexamethasone 0.15 mg/（kg·d）,4 times/d,intravenous drip,for 4 days.In the control group,the plan was the same with the treatment group in the uses of celtriaxone sodium,penicillin G and dexamethasone,while combined with chloramhenicol 75 mg/（kg·d）,qd,intravenous drip.The treatment course was 3 weeks in the two groups.Results In the treatment group,the average time of normal temperature was 2.75 days,3.75 days in the control group（t=1.56,P0.05）.The number of patients with normal cerebrospinal fluid was 16 after 2 weeks in the treatment group,and 15 in the control group（χ2=0.649,P0.05）.There was 1 case with complication in the treatment group,2 cases with complications in the control group（χ2=0.358,P0.05）.Conclusion The therapy of purulent meningitis with undetermined pathogenic bacteria with penicillin G and ceftriaxone sodium is effective,need not plus chloramhenicol,and the price is cheat.
