摘要
目的探索新疆地区造口患者院外延续护理的运作模式,提升出院造口患者的护理质量。方法从造口相关科室选择3名护理骨干,对154例出院造口患者采取电话随访、基于网络平台的健康教育、开设造口护理专科门诊的方式开展院外延续护理。结果出院1年后,患者造口相关知识、态度及其生活质量显著高于出院前;常见造口并发症低于出院前。96.11%患者对院外延续护理服务满意。结论院外延续护理可提高新疆地区造口患者的认知水平和生存质量,降低造口远期并发症。
Objective To establish a model for ensuring continuity of nursing care for discharged patients with stomas in Xinjiang province,and to improve nursing quality of patients with stomas.Methods Three nurses were chosen from stoma-related departments to provide continued nursing care to 154 discharged stoma patients by using telephone follow-up,web-based health education,and nursing clinic.Results Compared with the baseline before discharge,patients′ stoma-related knowledge,attitude,and quality of life were significantly improved after 1-year provision of continued nursing care,and stoma complication rates were decreased.Strictly,96.11% patients were satisfied with provision of continued nursing care.Conclusion Provision of continued nursing care to stoma patients in Xinjiang can improve their perception and quality of life,and reduce incidence of long-term complications.
