动脉粥样硬化斑块中牙周致病菌的检测 被引量:4

摘要 目的探讨牙周炎的程度是否影响动脉粥样硬化斑块中牙周致病菌的检出率。方法 29例患有不同程度牙周炎患者,行冠状动脉搭桥手术时,取动脉粥样硬化斑块及术前取龈下菌斑,采用chelex-100法提取细菌DNA,以聚合酶链反应法检测牙龈卟啉单胞菌、伴放线放线杆菌、福赛斯坦氏菌3种牙周炎相关致病菌。29人分为2组,重度牙周炎组16人,轻度牙周炎组13人。结果 2组患者冠状动脉粥样硬化斑块中,3种牙周炎相关致病菌的检出率分别为:牙龈卟啉单胞菌重度牙周炎组37.50%,轻度牙周炎组15.38%(P<0.05);伴放线放线杆菌重度牙周炎组18.75%、轻度牙周炎组未检出(P<0.01);福赛斯坦氏菌重度牙周炎组25.00%,轻度牙周炎组7.69%,(P<0.05)。同一患者冠状动脉粥样硬化斑块及龈下菌斑中同时检出3种病菌的一致率分别为:牙龈卟啉单胞菌重度牙周炎组5例(31.25%),轻度牙周炎组2例(15.38%);伴放线放线杆菌重度牙周炎组2例(12.50%),轻度牙周炎组未检出;福赛斯坦氏菌重度牙周炎组3例(18.75%),轻度牙周炎组1例(7.69%)。结论牙周致病菌在冠心病合并严重牙周炎患者的粥样硬化斑块中,检出率高于患轻度牙周炎的冠心病患者。 Objective To compare the detection rate of periodontol bacteria in coronary atherosclerotic plaques between severe periodontitis and mild periodontitis.Methods The coronary atherosclerotic plaques and subgingival plaques from 29 patients for coronary artery bypass surgery were collected,and the DNA from the bacteria of the plaques was extracted using the chelex-100 method.Three kinds of periodontal bacteria Porphyromonas gingivalis(P.g),Actinobacillus actinomycetemcomtans(A.a) and Tannerella forsythensis(T.f) were determined by PCR.There was 16 patients with severe periodontitis and 13 with mild periodontitis.Results In coronary atherosclerotic plaques,the detection rates of P.g,Aa and T.f were higher in severe periodontitis than in mild periodontitis(P0.05).The concordant presence of the same periodontal bacterium in subgingival plaques and coronary atherosclerotic plaques in the same patient was P.g: 5 cases in severe periodontitis,2 in mild periodontitis;A.a: 2 in severe periodontitis,O in mild periodontitis;T.f: 3 in severe periodontitis,1 in mild periodontitis.Conclusion The detection rate of periodontal bacteria in coronary atherosclerotic plaques in severe periodontitis is higher than that in mild periodontitis.
