临床血标本中产ESBLs大肠埃希菌耐药性检测 被引量:14

ESBLs-producing escherichia coli isolates from blood samples and their resistance

摘要 目的了解临床血标本中产ESBLs大肠埃希菌检出率及其对常用抗菌药物的耐药性。方法 CLSI表型确证试验纸片增强法检测产ESBLs大肠埃希菌;采用药敏分析仪对产ESBLs大肠埃希菌进行抗菌药物的敏感性试验。结果 44株血标本分离大肠埃希菌中,检出产ESBLs菌株20株,占45.5%;科室分布较为分散,其中神经内科、泌尿外科及血液内科各分离出3株,均占15.0%;产ESBLs菌株对亚胺培南、哌拉西林/他唑巴坦100.0%敏感,对氨苄西林、哌拉西林、头孢唑林、头孢曲松、头孢他啶、头孢噻肟、头孢吡肟、氨曲南的耐药率均为100.0%;对阿米卡星、头孢西丁的耐药率分别为5.0%、10.0%,对其他抗菌药物的耐药率均>50.0%。结论临床血标本分离大肠埃希菌中,产ESBLs菌株发生率较高,并显示多药耐药性。 OBJECTIVE To investigate Extended-spectrum β-lactamases (ESBLs) producers in Escherichia coli isolates from blood samples and their drugs resistance. METHODS ESBLs producing strains were determined by phenotypic confirmatory test according to CLSI 2008, and antimicrobial susceptibility testing was performed using Microscan Walkaway. RESULTS Of 44 isolates of E. coli, 20 strains (45.5 %) were detected as ESBLs producers with distribution of different wards. All of ESBLs-producing strains were susceptible to imipenem and piperacillin/ tazobactam, and the resistance rates of them to amikacin, cefoxitin were 10.0% and 5.0%, respectively. The resistance rates of those strains to ampicillin, piperacillin, cefazolin, ceftriaxone, ceftazidime, cefotaxime, cefepime, aztreonam were 100.0%, and more than half of them were resistant to the rest of antimicrobial agents. CONCLUSION ESBLs-producing strains are highly prevalent in in E. coli isolates from blood samples and are resistant to most of antimicrobial agents.
