摘要
目的探讨原发性脑淋巴瘤的CT、MRI表现与病理特征的相关性,以提高诊断准确率。方法回顾分析19例经活检或手术病理证实的脑淋巴瘤的CT与MRI表现,并与病理资料对照研究。结果 19例患者单发11例,多发8例,共发现32个病灶,多发病灶常呈区域性分布。病灶常位于脑表浅部位和近中线部位。CT表现为边缘清楚的等或稍高密度;T1WI呈等或稍低信号,T2WI多数呈等或稍低信号,所有病灶均明显强化,其中26个较均匀,6个病灶有小囊状或裂隙状无强化区,病灶形态呈"握拳样"或"团块样",水肿和占位效应程度不一。病理上均为弥漫性大B细胞淋巴瘤。结论病理形态基础决定脑淋巴瘤CT、MRI表现有一定特征,认真分析病灶部位、分布、形态、数目、密度、信号及强化等影像表现,可明显提高诊断准确率,有利于临床治疗及预后。
Objective To explore the connection between the CT and MR features of primary intracranial lymphoma and the pathological manifestation,and improve the accuracy of the diagnosis.Methods 19 patients with primary intracranial lymphomas confirmed by surgery or biopsy were respectively analyzed,including the CT and MR features and the pathological data.Results 11 cases had single lesion,8 cases had multiple lesions.32 lesions were found in 19 cases L？multiple lesions distributed regionally.most lesions were located near the surface or middle line of brain.On CT scan appearances,most lesions were isodensity or slightly hyperdensity masses with the well definite margin,on T1WI,the lesions showed iso or hypointense.on T2WI,most lesions presented iso or midly hypointense.all lesions were enhanced markedly.26 lesions presented homogeneous enhancement,6 lesions presented microcystic or slitshaped noenhanced areas,the lesions like ＂closed fist＂ or ＂lump＂,the lesions had the different extent in edema and space occupation effect.all the cases were diffuse large B cell lymphoma.Conclusion The pathologic morphologic basis of primary intracranial lymphoma determined the CT and MR characteristic manifestation,through careful analysis of the imaging appearances such as location,distribution,morphlogy,number,density,signal and enhancement,it was possible to improve the accuracy of the diagnosis,and it was help for clinical treatment and prognosis.
