摘要
目的观察小剂量红霉素治疗婴儿胃食管反流(GER)相关性肺炎的临床疗效。方法将60例1～6月确诊为GER相关性肺炎患儿,随机分为治疗组33例和对照组27例,对照组的肺炎采用常规治疗和改变体位及少量多次喂养,治疗组在对照组的基础上加用小剂量红霉素,3～5mg/(kg.d),1次/d,连用5d。结果治疗组的GER总有效率为96.97%,对照组为40.47%,χ2=23.12,P<0.05,差异有统计学意义;治疗组的肺炎总有效率为90.9%,对照组为66.7%,χ2=5.45,P<0.05,差异有统计学意义;咳嗽、气喘、肺部啰音消失时间及住院时间治疗组均较对照组缩短,t分别为5.21,3.08,2.73,4.74,P均小于0.05,差异有统计学意义;随访6个月治疗组再患肺炎为9.1%,对照组为29.6%,χ2=4.18,P<0.05,差异有统计学意义。结论小剂量红霉素治疗婴儿GER相关性肺炎疗效显著,无明显不良反应,可以降低再患肺炎的风险。
Objective To observe clinic effect of small doses of erythromycin in infant with pneumonia correlating gastroesopha- geal reflux. Methods Sixty infants aged 1 -6 months who were diagnosed pneumonia correlating gastroesophageal reflux were di- vided two groups （33 cases in the treat group and 27 cases in the control group） at random. They were given standard therapy and changed body position as well as repeated small doses feeding in the control group while additional small doses of erythromycin （ 3 - 5 mg . kg-1.d - 1, one time every day for 5 days） in the treat group. Results The effective rates was 96.97 % in the treat group and 40.47% in the other group, there was a remarkable difference between the two groups （X2 = 23.12, P 〈 0.05 ） ;the time of cough and wheeze, assimilative time of murmur in the lung and hospitalized time shortened in the treatment group were significantly different from those in control group （ t were 5.21,3.08,2.73,4.74 separately and P 〈 0.05 without exception） ; repeated pneumonia was 9.1% in the treat group versus 29.6% in the other group after 6 months follow-up, the statistical difference was significant. Conclusion The therapy of small doses of erythromycin had high effective in infant with pneumonia correlating gastroesophageal reflux and no obvious side effect,it can reduce the risk of repeated pneumonia.
