摘要
目的了解新型农村合作医疗(新农合)定点医疗机构合理用药现状,促进合理用药水平的提高。方法随机抽取河南省某县县乡两级新农合定点医疗机构的门诊处方,对处方药品种数、处方金额、抗菌药物、针剂使用比例等信息进行统计分析。结果县、乡定点医疗机构单张处方用药最大值分别为5和9种,平均每张处方用药分别为1.97和2.68种。县医院含抗菌药物的处方共52张,其中67.3%使用1种抗菌药物,28.8%使用2种抗菌药物,使用3种和4种抗菌药物的处方各占1.9%。乡镇卫生院使用抗菌药物的处方共有35张,其中91.4%使用1种抗菌药物,5.7%使用2种抗菌药物,2.9%使用3种抗菌药物。县医院含针剂的处方5张,乡镇卫生院32张,针剂的使用率分别为3.4%和23.5%。县医院单张处方最高费用为380.5元,最低为0.2元,平均费用84.86元;乡镇卫生院单张处方最高费用为434.0元,最低为0.4元,平均费用50.58元。结论样本地区县、乡两级新农合定点医疗机构用药情况总体较好,在降低处方费用及基层医疗机构处方用药数量方面仍有待加强。改善农村地区合理用药的现状,需要完善医疗机构内部管理,加强医务人员合理用药知识与技能的培训,完善合理用药制度建设,培养农民合理用药意识等多方面的努力。
Objective To investigate the status of drug use in new rural cooperative medical institutions and promote the level of rational drug use. Methods Randomly selected the out-patient prescriptions in county and township new rural cooperative medical institutions in a county in Henan province,analyzed the average kinds of drugs prescribed in each prescription,the average cost,and the percentage of outpatients that received antibacterial drugs,injections. Results Maximum numbers of drugs per prescription were 5 for county hospital and 9 for township hospital,and the mean numbers were 1.97 and 2.68,respectively.There were 52 prescriptions including antibacterial drugs in county hospitals,and 67.3% contained one kind of antibacterial drugs,28.8% with two kinds,1.9% of them consisted of three or four kinds of antibacterial drugs.While in township hospital,the number of prescriptions including antibacterial drugs was 35,91.4% with one,5.7% and 2.9% contained two and three kinds of those drugs respectively.The number of prescriptions with injections was 5 for county hospital and 32 for township hospital,meanwhile,the utilization rates of injections were 3.4 and 23.5,respectively.The maximum of per prescription expense of county hospitals was 380.2 yuan,minimum of that was 0.2 yuan,and the mean was 84.86 yuan,while the numbers of the township hospitals were 434,0.4 and 50.58 yuan,respectively. Conclusion The status of drug use in sample medical institutions is general good,reduce prescription costs and quantities of prescription drugs in township level medical institutions should be strengthened.Improve the level of rational drug use in rural areas needs many efforts,which including the followings,improve the internal management of medical institutions,enhance knowledge and skills training of the rural doctors,cultivate awareness of rational use of drugs of the farmers and so on.
