摘要
目的评价显微超声技术联合先锋锉疏通钙化根管的临床效果。方法对常规方法无法疏通的115颗患牙共187个钙化根管,采用显微超声技术探查和定位根管口,去除根管上端钙化物后用先锋锉结合乙二胺四乙酸凝胶疏通根管,评价根管疏通效果。结果 103颗患牙共168个根管疏通成功,患牙疏通成功率为89.6%,钙化根管疏通成功率为89.8%。根管上1/3钙化的疏通成功率为93.1%;根管中下2/3钙化的疏通成功率为91.6%;根管全段闭锁钙化的疏通成功率为50%,低于上述两种部分钙化的根管疏通成功率,差异有统计学意义(P<0.05)。2个未疏通到根尖孔的根管发生了根管偏移。无根管侧穿及器械分离发生。结论显微超声技术联合先锋锉能安全有效地疏通钙化根管。
Objective To evaluate the clinical effect of ultrasonic instruments combined with C-Pilot files under dental operating microscope（DOM） in the management of calcified root canals.Methods DOM,ultrasonic instruments and C-Pilot files were applied on 115 teeth of 100 patients with 187 calcified root canals which can not be negotiated with conventional method.Ethylene diamine tetra acetic acid（EDTA） gel was used to decalcify calcified root canals.Then the success rate was recorded.Results One hundred and three teeth were successfully treated with a success rate of89.6%.One hundred and sixty-eight calcified root canals were successfully treated with the success rate was 89.8%.Five completely calcified root canals were successfully treated with the success rate was 50%,which was inferior to that of partial calcified root.There was no perforation or instruments separation in all the cases.However,two root canals transportation were observed in failed cases.Conclusion Combined use of ultrasonic instruments and C-Pilot files under DOM is proved to be an effective and safe way in the management of calcified root canals.
