摘要
目的了解上海社区40-79岁男性居民下尿路症状（LUTS）的发生情况、年龄分布及其变化规律。方法运用整群随机抽样的方法抽取社区男性居民1 539人,采用国际前列腺症状（IPSS）评分表调查社区男性LUTS的发生情况。结果各年龄组LUTS发生情况为：40-49岁为178人,IPSS评分为〔0（1.0）〕分,其中83人（46.6%）为轻度（IPSS 1-7分）LUTS,3人（1.7%）为中度（IPSS 8-19分）LUTS;50~59岁为446人,IPSS评分为〔1.0（2.0）〕分,其中269人（60.3%）为轻度LUTS,27人（6.1%）为中度LUTS,3人（0.6%）为重度（IPSS 20-35分）LUTS;60~69岁为629人,IPSS评分为〔2.0（4.0）〕分,其中424人（67.4%）为轻度LUTS,74人（11.8%）为中度LUTS,10人（1.6%）为重度LUTS;70-79岁为286人,IPSS评分为〔4.0（7.0）〕分,其中158人（55.2%）为轻度LUTS,70人（24.5%）为中度LUTS,18人（6.3%）为重度LUTS;各年龄组间IPSS评分比较差异有统计学意义（P〈0.05）。男性LUTS严重程度与年龄呈正相关（r=0.32,P〈0.001）。结论 LUTS在≥40岁上海社区男性中普遍存在,而且随着年龄的增大,发病率增高,LUTS逐渐加重。
Objective To estimate the prevalence,age distribution and characteristics of lower urinary tract symptoms（LUTS） among Shanghai community adult men from forty to seventy-nine.Methods A total of 1539 male community residents were enrolled through cluster random sampling and questionnaired about the LUTS situations using the International Prognostic Scoring System（IPSS）,All subjects enrolled were divided into four groups（10 years for each stratum）.Results The occurrence rate and severity of LUTS for each group were as follows.The average IPSS score was（0（1.0）） among the 178 men aged 40~49,83（46.6%） of them showed mild LUTS（IPSS,1~7）,3（1.6%） showed moderate LUTS（IPSS,8~19）;average IPSS score was（1.0（2.0）） among the 446 men aged 50~59,269（60.3%） of them showed mild LUTS,27（6.1%） showed moderate LUTS,and 3（0.6%） showed severe LUTS（IPSS,20~35）;average IPSS score was（2.0（4.0）） among the 629 men aged 60~69,424（67.4%） of them showed mild LUTS,75（11.8%） showed moderate LUTS,10（1.6%） showed severe LUTS;average IPSS score was（4.0（7.0）） amont the 286 men aged 70~79,158（55.2%） of the showed mild LUTS,70（24.5%） showed moderate LUTS,18（6.3%） showed severe LUTS;IPSS scores were found to increase significantly with age among these subjects（P〈0.05）.In comparison with the IPSS scores of the 40~49 group,the 50~59,60~69,and 70~79 groups all showed significant differences.The severity of LUTS among different age groups was correlated with age（r=0.32,P〈0.001）.Conclusion LUTS are relatively common among Shanghai community males aged more than 40.LUTS incidence rate and IPSS scores increased with age.
