摘要
目的探讨320排动态容积CT(320-slice dynamic volume CT,320-DVCT)评价壁冠状动脉-心肌桥(MCA-MB)的形态学特征、准确性及其临床应用价值。方法 85例可疑冠心病住院患者行心脏320-DVCT并冠脉CTA,同期行冠状动脉造影术(CAG),分析MCA-MB影像学特点。结果 320-DVCT对MCA-MB的检出率为30.6%,其中82.4%出现在左前降支中、远段,平均长度为(13.38±5.06)mm,MB平均厚度为(3.61±2.53)mm。MCA-MB合并邻近血管动脉粥样硬化发生率为96.2%。CAG的MCA-MB的检出率为8.2%,320-DVCT对MCA-MB的检出率明显高于CAG。结论 320-DVCT可准确判断MCA-MB的结构特点;临床怀疑冠心病者MCA-MB与动脉粥样硬化有一定相关性。320-DVCT是一种无创、准确诊断和评价MCA-MB的方法。
Objective To evaluate the morphological assessment, accuracy and clinical significance of 320 - slice dynamic volume CT （320 -DVCT） in diagnosis of mural coronary artery -myocardial bridges （MCA- MB）. Methods The 320 - slice DVCT scan and coronary angiography （CAG） were performed in 85 patients suspected of coronary artery disease （CAD） for evaluation of the efficacy of DVCT in MCA - MB diagnosis. Results The detection rate of MCA - MB with 320 - slice DVCT was 30. 6%, among which 82.4% were located in middle and distal segment of left an- terior descending artery. The mean length and thickness of MB were （ 13.38±5.06）mm and （3.61 ±2. 53）mm, respectively. The incidence of atherosclerosis of MCA - MB adjacent vessels was 96. 2%. Meanwhile, the detection rate of MCA - MB with CAG was 8.2%, which was significantly lower than that with DVCT （ P 〈 0. 05 ）. Conclusion As a new non - invasive approach of assessing MCA - MB, 320 - slice DVCT provides sufficient accuracy and reliability. Furthermore, MCA - MB in patients suspected of CAD is correlated with atherosclerosis.
