不同浓度地佐辛用于小儿术后自控镇痛的临床研究 被引量:7

Clinical study on self-controlled analgesia with dezocine of various concentration after pediatric operation

摘要 目的探讨地佐辛在小儿单侧腹股沟斜疝修补术术后静脉自控镇痛(PCIA)的有效性和安全性。方法选择择期全麻下小儿单侧腹股沟斜疝修补术90例,随机分成A、B、C3组(每组30例)。A组:地佐辛0.6mg·kg-1+盐酸昂丹司琼0.1mg·kg-1+0.9%氯化钠注射液至100ml;B组:地佐辛0.8mg·kg-1+盐酸昂丹司琼0.1mg·kg-1+0.9%氯化钠注射液至100ml;C组:地佐辛1.0mg·kg-1+盐酸昂丹司琼0.1mg·kg-1+0.9%氯化钠注射液至100ml。分别观察并记录患儿启用镇痛泵后2、4、8、12、24和48h的疼痛评分;并记录PCIA期间不良反应的发生情况。结果 A组的VAS评分为基本满意;与A组比较,B组和C组效果更优(P均<0.05);B组与C组VAS评分比较差异无统计学意义(P>0.05)。A组和B组患者嗜睡发生率稍低。结论地佐辛用于PCIA可达到满意的镇痛效果,其不良反应发生率低,安全范围大。0.8mg·kg-1的地佐辛是小儿PCIA一个较合适的剂量。 Objective To investigate the effectiveness and reliability of different dose in the postoperative patient-controlled intravenous analgesia (PCIA) of half inguinal hernia repair in children. Methods A total of 90 children underwent half inguinal hernia repair operation under general anesthesia were randomly divided into group A、B、C(n=30 each).Dezocine of different dose (0.6 mg·kg-1 in group A,0.8 mg·kg-1 in group B,1.0 mg·kg-1 in group C) and ondansetron hydrochloride (0〈1 mg·kg-1) plus normal saline respectively were made 100 ml solution in three groups.The analgesia pump was switched on for self-controlled analgesia after waking up from anesthesia.The visual analogue scale (VAS) were used to evaluate the analgesic effect at 2,4,8,12,24 and 48 h after enabling analgesic pump,and the adverse effects associated with PCIA were recorded during PCIA. Results VAS scores showed that the satisfactory pain relief was attained in group A,but the analgesic effect was not as good as group B and C (all P〈0〈05),while there was no significant difference in group B and C (P〉0〈05).The incidence of lethargy was slightly lower in group A and B. Conclusions PCIA with dezocine has a good analgesic effect and a low side effect. The dose of 0.8 mg·kg-1 dezocine is suitable dosage in PCIA for children
