ER、PR和C-erbB-2在甲状腺癌中的表达及意义

摘要 目的研究ER、PR、C—erbB-2在甲状腺癌组织中的表达情况,探讨它们的相互关系及临床意义。方法用免疫组化的方法检测ER、PR、C—erbB-2在甲状腺癌、甲状腺腺瘤和甲状腺正常组织中的表达,分析它们在不同病变组织中的表达状况以及与甲状腺癌临床病理参数的关系。结果甲状腺癌组织中ER、PR、C—erbB-2的阳性表达率(58.5%、71.7%、41.5%)明显高于甲状腺腺瘤(36.O%、38.0%、20.0%)和甲状腺正常组织(4.0%、8.0%、4.0%)(P〈0.05)。甲状腺癌患者中年龄〈40岁组ER、PR阳性率(76.5%、91.2%)远高于≥40岁年龄组(26.3%、36.8%),女性ER、PR阳性率(69.8%、83.7%)高于男性(10%、20%);乳头状癌阳性率远高于其它分型(P〈0.05)。ER、PR在无淋巴结转移患者的阳性表达率高于有淋巴结转移者(P〈0.05),而C—erbB-2的表达无淋巴结转移者远低于有淋巴结转移者(P〈0.05);C—erbB-2的表达与肿瘤大小、性别、年龄无相关性(P〉0.05)。结论甲状腺癌、甲状腺瘤、甲状腺正常组织中均可检测到ER.PR、C—erbB-2的阳性表达,以甲状腺癌最高,提示女性激素可能是诱发甲状腺癌发生的重要因素;ER、PR、C—erbB-2在乳头状癌阳性率远高于其它分型,提示乳头状癌可能是女性激素依赖性肿瘤;在甲状腺癌中,ER、PR与C—erbB-2的表达呈负相关。联合检测ER、PR、C—erbB-2为进一步了解甲状腺癌的发生发展,判断甲状腺癌的恶性程度、预后及有无淋巴结转移有重要意义。 Objective To study the expression of ER, PR andC -erbB -2 in thyroid carcinoma,to explore the relations among them and their clinical values. Methods Normal thyriod tissues, thyroid adenoma and thyroid carcinoma were evaluated for the expression of ER, PR and C -erbB -2 by immunohistochemistry, the levels of these proteins in tissues with different types of lesion and their correlation with clinicopathological parameters of thyroid carcinoma were analyzed. Results Compared with normal thyroid tissues and thyroid adenoma, the expression rates of ER, PR and C - erbB - 2 of thyroid carcinoma increased obviously, there was significant difference between thyroid carcinoma and them ( P 〈 0. 05 ). The patients with thyroid carcinoma,the expression rates of ER and PR in the group with age 〈40 increased obviously compared with the group with age ≥40, there was significant difference between them (P 〈 0. 05 ). the expression rates of ER and PR in famale were higher than them in male, there was significant difference between them ( P 〈 0. 05 ). The positive rates of ER, PR andC - erbB - 2 in papillocarcinoma were higher than them in other thyroid carcinoma pathological types ( P 〈 0. 05 ). the positive rates of ER and PR in patients with no - lymph node metastasis were higher than them with lymph node metastasis ( P 〈 0.05 ). The positive rate ofC - erbB - 2 in patients with no - lymph node metastasis were lower than that with lymph node metastasis ( P 〈 0. 05 ). The expression of ER, PR and C - erbB - 2 were no associated with the sex, age of patients and the size of tumor(P 〉 0. 05 ). Conclusion The positive expression of ER, PR andC -erbB -2 were detected in normal thyroid tissues, thyroid adenoma and thyroid carcinoma. The expression in thyroid carcinioma was highest among them. Female sex hormone was probably a improtant agent induced the devolpment of thyroid carcinoma. It was close associated with the devolpment of thyroid carcinoma. The positive rates of ER, PR andC - erbB -2 in papillocarcinoma were higher than them in other thyroid carcinoma pathological types. Papillocarcinoma was probably a female sex hormone - dependent tumor. The expression of ER and PR were negative correlated withC - erbB - 2 in thyroid carcinoma. Simultan - eously examing the expression of ER, PR andC - erbB - 2 may have important effect on further investigation the mechanism of the devolpment of thyroid carcinoma, judgement the degree of malignancy, prognosis and with or without lymph node metastasis in thyroid carcinoma.
