摘要
目的:调查上海市二、三级医院2007-2009年抗菌药物应用情况,为合理应用抗菌药物提供依据。方法:对上海市12家二级医院和14家三级医院2007-2009年的抗菌药物用药数据,包括处方量、用药金额、均次费用、用药频度(DDDs)、复合年增长率进行统计分析。结果:二级医院抗菌药物用药金额占总用药金额的比例显著高于三级医院(22.11%vs15.05%,P<0.01)。二、三级医院抗菌药物的用药金额、处方量和均次费用逐年增长,并且特殊管理类抗菌药物用药金额占比最高。第二、三代头孢菌素和喹诺酮类药物的用药金额居前3位,头霉素类药物用药金额增长最为迅速。用药金额排名前10位的抗菌药物中有多种β-内酰胺类药物,但各药品的复合年增长率有所不同。二、三级医院DDDs排名前10位的品种大多数相同,DDDs的排序与用药金额的排序不一致。结论:二、三级医院的抗菌药物应用情况有部分差异。应加强抗菌药物管理制度的落实,提高抗菌药物合理使用水平。
Objective:To investigate the utilization of antibacterials in second-calss and third-class hospitals in Shanghai,so as to provide a base for rational use of antibacterials.Methods:The numbers of prescriptions,consumption sum,average cost of each prescription,use frequency,compound annual growth rate(CAGR) of antibacterials in 12 second-class hospitals and 14 third-class hospitals in Shanghai during 2007-2009 were statistically analyzed.Results:The constituent rate of consumption sum of antibacterials was significantly higher in the second-class hospitals than that in the third-class hospitals(22.11% vs 15.05%,P0.01).The consumption sum,number of prescriptions and average cost of each prescription in second-class and third-class hospitals were increasing on an annual basis;and the constituent rate of consumption sum of antibacterials of special management class was highest.The second and third generation cephalosporins and quinolones ranked top 3 in consumption sum.The consumption sum of cephamycins had the fastest increase.β-lactams were the dominant in the top 10 drugs of consumption sum,but the CAGR of each drug was different.Most of the antibacterials which ranked top 10 in DDDs were the same in second-class and third-class hospitals,and the rank of DDDs was inconsistent with that of consumption sum.Conclusion:There was some difference in the utilization of antibacterials between the second-class and the third-class hospitals.The management of antibacterials should be strengthened,and attention should be paid to the clinical rational use of antibacterials.
