术中超声联合神经电生理技术在功能区肿瘤中的作用研究

Intraoperative Ultrasound and Electrocorticography Monitoring in Resection of Eloquent Brain Tumor:Analysis of 38 Cases

摘要 目的探讨在联合术中超声和神经电生理监测在功能区肿瘤中的作用和意义。方法对38例功能区肿瘤联合术中超声和脑皮层电图监测进行显微外科手术,观察手术效果及并发症发生率。结果术中超声均能精确定位;术中全切除26例,次全切除12例,术后神经功能改善,无明显并发症,无死亡。结论通过术中超声设计解剖定位,确认术中病灶周围血管及重要结构毗邻关系,通过术中神经电生理实时监测,功能定位、指导手术操作、降低手术风险、提高手术疗效方面有确切的临床价值。 Objective To evaluate the applied value of intraoperative ultrasound and electrocorticography monitoring in resection of eloquent brain tumor.Methods A series of thirty-eight cases of intracranial tumors were resected under intraoperative ultrasound united by electrocorticography monitoring.The clinical data and postoperative outcome were analyzed.Results All the 38 lesions were accurately defined by intraoperative ultrasound.Total removal was achieved in 26 cases and subtotal resection in 12 cases.Neurological symptoms improved and no severe complications appeared.No patients died also.Conclusions Intraoperative ultrasound united by electrocorticography monitoring can accurately locate the eloquent tumor and retrieve the brain shift.It can improve the total removal and depress the mortality and disabled rate.
