磁共振全身弥散加权成像在体部肿瘤筛查中的应用 被引量:28

摘要 目的评价磁共振全身弥散加权成像(WB-DWI)在体部肿瘤筛查中的应用价值。资料与方法分析30例正常志愿者和54例(221处病灶)经手术病理证实的体部肿瘤的WB-DWI影像表现特征,均采用1.5TMR体部线圈进行扫描,观察正常志愿组的WB-DWI影像表现,利用三维最大密度投影(3DMIP)及黑白翻转技术重建出类PET图像。测量病例组221处病灶的表观弥散系数(ADC值),并进行良、恶性病变的ADC值对比。结果正常志愿组WB-DWI可清晰地显示颅脑、甲状腺、脾、肾、膀胱、椎间盘、前列腺、子宫及淋巴结等组织结构,呈高信号;病例组221处病灶在WB-DWI呈高信号,且均经常规MRI证实,恶性肿瘤平均ADC值[(1.04±0.46)×10-3mm2/s],低于良性肿瘤平均ADC值[(2.53±0.43)×10-3mm2/s](t=19.12,P<0.05)。结论 WB-DWI在体部肿瘤筛查中有一定的临床价值,且对良、恶性肿瘤有一定的鉴别价值。 Purpose To evaluate the efficacy of MR whole body diffusion weighted imaging (WB-DWI) in screening body neoplasms. Materials and Methods A total of 30 healthy volunteers and 54 patients (221 lesions) with histologically confirmed tumors were enrolled in this study. WB-DWI examination was performed on GE 1.5T MR scanner using embedded body coil. The WB-DWI appearances of healthy volunteers were assessed using three-dimensional maximum intensity projection (3D MIP) reformatted images. The ADC values of 221 lesions in 54 patients were measured and ADC values of benign and malignant lesions were compared. Results In 30 healthy volunteers, brain, thyriod, spleen, kidney, urinary bladder, intervertebral discs, prostate, uterus and lymph nodes showed high signal intensity on WB-DWI. 221 lesions in 54 patients showed high signal on DWI, all of which were confirmed by conventional MRI examination. The mean ADCs of malignancy were significantly lower than those of benign tumors [(1.04±0.46)×10-3 mm2/s vs (2.53±0.43)×10-3 mm2/s](t=19.12, P0.05). Conclusion WB-DWI is excellent in screening body neoplasms and shows important application in differentiating benign and malignant tumors.
