摘要
目的应用Meta分析评价肿瘤坏死因子(TNF)--α308位点多态性与胃癌易感性的关系。方法通过计算机检索PubMed和中文期刊网(CNKI)数据库,收集2001～2010年国内外公开发表的关于TNF-α基因(TNF-Α)多态性与胃癌的关系研究的文献。应用Review Manager 5.0软件进行统计学分析,二分类变量用比值比(OR)为效应量,并计算95%可信区间(95%CI)。根据异质性检验,分别应用固定效应模型或随机效应模型进行分析处理。结果对于TNF--α308位点,研究中共纳入28篇文献,病例组共5 069人,对照组共有8 387人,经异质性检验,各研究间有统计学异质性(P=0.003)。随机效应模型分析结果显示-308位点等位基因的分布在2组中差异有统计学意义(OR=1.17,95%CI:1.04～1.33,P=0.009)。在地区及肿瘤位置的基础上进行亚组分析显示:在西方人群中病例组和对照组分析差异有统计学意义(OR=1.20,95%CI:1.05～1.37,P=0.007);在东亚人群中病例组和对照组分析差异无统计学意义(P=0.52);在非贲门癌人群中病例组和对照组分析差异无统计学意义(P=0.14)。结论在西方人群中,TNF--α308位点可能是增加胃癌患病风险的危险因素。
Objectives Ameta-analysis were used to assess the association between tumor necrosis factor-α（TNF-α） gene-308（G/A） polymorphisms and gastric cancer（GC） susceptibility.Methods We searched the reference from PubMed and CNKI database between the year 2001~2010 published on the association between the TNF-α gene（TNF-α） polymorphism and gastric cancer（GC） susceptibility.The Revman 5.0 software was used for meta-analysis.Dichotomous outcomes were expressed as odds ratios（OR） with 95% confidence intervals（CI）.According to the result of the heterogeneity test,we used the fixed effect model or rondom effect model to carry on the merge analysis respectively.Results There were 5069 GC patients and 8387 controls for TNF-α-308.Overall,allele contrast（A vs.G） of TNF-α-308 polymorphism produced significant results in worldwide populations [Pheterogeneity=0.003,random-effects（RE） OR=1.17,95%CI：1.04~1.33,P=0.009].We also performed subgroup analysis based on ethnicity（west,east Asian） and tumor location [non-cardia gastric cancer（NCGC）].Significant association was observed in west subgroup analysis（OR=1.20,95%CI：1.05~1.37,P=0.007）.No significant association was observed in east Asians and NCGC subgroup analysis（P=0.52,P=0.14）.Conclusion The polymorphism of the TNF-α-308 locus would be a risk factor for GC especially in west populations.
