摘要
目的回顾性分析16例不典型乳腺癌误诊病例。方法跟踪门诊超声诊断为乳腺纤维腺瘤的住院患者,对术中冰冻切片确诊为乳腺癌的16例进行分析,结合术前病历及术后病理分析误诊乳腺癌与正常乳腺纤维瘤的超声细微差别。结果经手术后病理证实均为乳腺癌的患者,其乳腺肿块超声图像可与正常乳腺纤维腺瘤表现类似,但结合患者年龄,肿块位置及血流信号具有差别,肿块内部回声不一致,微钙化灶可对肿块提出恶性可能的诊断。结论乳腺恶性肿块复杂多样,早期病例和不典型病例单凭超声图像容易误诊,需结合患者病历、肿块具体特征、血流速度及阻力、微钙化灶有无进行综合分析,钼靶和MRI、穿刺活检对超声诊断也有帮助。
Objective To discuss the experience of misdiagnosis of untypical breast cancer.Methods The patient in OPD diagnosed as fibroadenoma was followed up and there were 16 cases who were diagnosed as cancer after FS.Their clinical materials were analyzed retrospectively.Results The ultrasound image of these patients was similar with fibroadenoma.The age,location,blood signal and microcalcification could be related with cancer.Conclusion The diagnosis of malignance tumor of breast was complex especially in early stage and untypical tumor.The age,location,blood signal and microcalcification should be compbined and considered with molybdenum target,MRI and biopsy.The operation should be taken as early as possible.
