摘要
目的了解东阳市不同健康人群中流感嗜血杆菌（haemophilus influenzae,Hi）的携带状况,探讨分离株在各型中的分布和对常用抗生素的耐药性。方法通过采集238份健康人群的咽拭子,将分离到的流感嗜血杆菌用生物学分型、血清学分型及脉冲场凝胶电泳（PFGE）分型等方法进行比较分析;采用K-B法对分离株进行12种常用抗生素的药敏试验。结果从238份样本中分离到21株Hi,携带率为8.82%,其中〉19周岁组和4~6周岁组携带率较高,分别为22.86%（8/35）和22.22%（8/36）,且6周岁以下年龄组的携带率有随着年龄增加而升高的趋势。本次调查分离到Hib血清型1株,携带率为0.42%（1/238）;其余20株均为不可分型的流感嗜血杆菌（NTHi）,携带率为8.40%（20/238）;生物学分型以Ⅲ型最多,占38.10%（8/21）;19株Hi可分为15个PFGE型,总体相似系数为48.8%,显示各分离株间变异程度大,同源性低;分离株对大多数常用抗生素敏感,但对复方磺胺甲噁唑的耐药率高达28.57%（6/21）。结论东阳市健康人群中Hi的携带率相对较低,且以不可分型株为主;各分离株的分子多态性可能是流感嗜血杆菌感染在东阳市没有形成暴发与流行的分子流行病学原因。今后仍需重点加强学龄前儿童携带Hi情况的研究,为Hi所致疾病的预防和控制提供科学依据。
Objective To investigate nasopharyngeal carriage for Haemophilus influenzae（Hi） among healthy residents in Dongyang City,Zhejiang,and explore the type distribution and antibiotic resistance of isolated strains.Methods A total of 238 throat swabs were collected from healthy residents in Dongyang,Zhejiang.The isolated strains of Hi were analyzed by bio-typing,serotyping and pulsed-field gel electrophoresis（PFGE） typing.Drug susceptibility tests were conducted for the isolated strains of Hi by Kirby-Bauer method.Results The carriage rate of Hi was 9.13%（21/238） for healthy residents.The positive rates of Hi were higher among residents aged over 19 years old（22.86%） or between 4 and 6 years old（22.22%）.One strain of Hib was confirmed and the others were non-determinable strains.Biotype Ⅲ（38.10%） was predominant by the assay of bio-typing.There were 15 patterns of PFGE typing with a similarity rate of 48.8%.The variety of PFGE patterns showed a wide diversity of genotypes.Isolated strains were sensitive to most antibiotics except for sulphamethoxazole/trimethoprim（resistance rate：28.57%）.Conclusion Hib carriage rate was low in healthy residents in Dongyang,Zhejiang.The variety of PFGE patterns suggested heterogeneous source of origin for isolated strains.
