摘要
目的比较乳腺癌保乳术辅助放疗与改良根治术辅助放疗的疗效。方法 210例乳腺癌患者按治疗方法的不同,分为保乳组(110例)和改良根治术组(100例)。保乳组术后放疗未行内乳照射,改良根治术组行内乳照射。比较两组术后疗效以及放疗的影响。结果保乳组的手术时间和术中出血量均明显少于改良根治术组(P<0.05);保乳组并发症的发生率(16.36%)与改良根治术组(22.00%)比较,差异无显著性(P>0.05);保乳组的乳房美容优良率(68.18%)明显高于改良根治组(34.00%),差异有显著性(P<0.01);两组术后放疗的皮肤反应分级的差异无显著性(P>0.05);两组局部复发率、远处转移率及生存率的差异无显著性(P>0.05)。结论乳腺癌患者保乳治疗并结合术后放疗,疗效较好,不需内乳照射,对放疗的皮肤反应、局部复发、远处转移和生存率均无影响。
[ Objective ] To investigate the efficacy of breast conserving surgery and radiotherapy on breast cancer patients. [Methods] A total of 210 cases breast cancer patients according to treatment methods were divided into breast-conserving group （110 cases） and modified radical mastectomy groups （100 cases）. Breast-conserving group did not take internal mammary irradiation after surgery, modified radical mastectomy group were taken internal mammary irradiation compared the efficacy and the impact of radiotherapy. [ Results] The surgery time and blood loss volume of breast-conserving group was significantly less than the modified radical mastectomy group （P 〈0.05）, there were no significant difference between the incidence of complications in breast-conserving group （16.36%） and modified radical mastectomy group （22.00%） （P 〉0.05）; Breast cosmetology good rate of breast-conserving group （68.18%） was significantly higher than the modified radical group （34.00%）, the difference was statistically significant （P 〈0.01）; radiotherapy skin reaction classification of two group had no significant difference postoperation （P 〉0.05）; local recurrence, distant metastasis rate and survival rate of two groups had no significant difference （P 〉 0.05）. [ Conclusion ] Breast cancer patients take breast-conserving combination with radiotherapy, have a better effect, do not need internal mammary irradiation, have no effect on radiotherapy skin reactions, local recurrence anddistant metastasis and survival rate.
