摘要
目的探讨良性前列腺增生(benign prostatichy perplasia,BPH)及前列腺癌(prostate carcinoma,PCa)磁共振成像(MRI)和磁共振灌注加权成像(PWI)表现以及灌注值,评价两者对BPH和PCa的诊断及鉴别诊断价值。方法对临床诊断为前列腺疾病的76例患者行常规MRI及PWI扫描,分析BPH及PCa的常规MRI表现及所获得时间-信号曲线(TIC),计算最大灌注斜率(SSmax)。结果 35例PCa及41例BPH的TIC均有明显的上升支,较之BPH组,PCa组TIC上升支较陡峭,峰值高,达峰时间短;PCa组的SSmax高于BPH组(P<0.05)。结论 MRI与PWI联合应用有助于PCa的诊断与鉴别诊断。
Objective To evaluate the value of MRI and PWI in diagnosing and differential diagnosing for benign and malignant prostate diseases.Methods MRI routine scanning and perfusion imaging were performed in consecutive 76 patients with clinically-proven prostatic disease.The signal time-intensity curve (TIC) and the maximum perfusion slope (SSmax) were obtained by Mean-Curve.Results In 35 patients with prostatic carcinoma and 26 patients with benign prostatic hyperplasia,the shapes of TIC in BPH group and PCa group are similar,both showing an obvious steep-up slope after the baseline.Compared with the BPH group,the upslope of TIC in PCa group is steeper,the peak is higher and the peak time is shorter.SSmax of prostatic carcinoma were higher than those of benign prostatic hyperplasia group (P0.01).Conclusions It is helpful to diagnosis and differential diagnosis of prostatic carcinoma when MRI and PWI are combined.
