摘要
目的探讨CT导向下射频消融(RFA)治疗肺部肿瘤的疗效与价值。方法总结25例肺部肿瘤进行了CT导向下射频消融治疗,术前常规CT导向下经皮穿刺活检或经支气管镜检查获取病理学诊断,鳞癌9例,腺癌15例,转移癌1例。共计25个肿瘤病灶,全部瘤体直径均<8 cm,其中22个病灶直径<4 cm,3个≥4 cm。结果全部病例手术顺利,无术中并发症发生。2例治疗后CT检查发现有少量气胸,少量痰中带血8例。术后分别在1个月和9个月CT复查,显示25个病灶体积均有不同程度的进行性缩小,肿瘤内部均出现大片不规则低密度区;有效率100%。3～9个月随访,总有效率96%。结论 CT导向下经皮穿刺RFA治疗肺部肿瘤是安全有效的微创技术,值得推广应用。
Objective To assess the therapeutic value of CT guided percutaneous radiofrequency thermal ablation（RFA） for lung neoplasms.Methods Twenty-five patients with pulmonary neoplasms were treated with RFA under the CT guidance.The CT guided percutaneous biopsies were performed to obtain the pathology before the procedures,squamous carcinoma 9 cases,adenocarcinoma 15 cases,and metastatic carcinoma 1 cases.Results The operations in all cases were successful,there were no complication happened during the procedures.Little amount of pneumothorax was found in 2 cases,a small quantity of bloody sputum were found in 8 cases.All the volume of lesions in 25 cases were diminished,effective rate were 100%.Follow-up after the procedures at 9 months,total effective rate were 96%.Conclusion CT guided percutaneous RFA for pulmonary neoplasms is a safe,effective,and minimal invasive procedure.
