摘要
目的 通过对Ⅰ型主动脉夹层外科治疗的回顾性研究,探讨对合并冠状动脉病变Ⅰ型主动脉夹层的外科治疗经验,以提高此类疾病的治疗效果.方法 2002年9月至2011年2月共治疗Ⅰ型主动脉夹层207例,其中73例合并冠状动脉病变,并需同期处理.冠状动脉搭桥20例,冠状动脉整合45例,冠状动脉成形8例.结果 73例合并冠状动脉病变的Ⅰ型主动脉夹层,2例死亡,死亡率2.74%.远期随访效果满意.结论 对Ⅰ型主动脉夹层合并冠状动脉病变的患者,术前完善检查,明确冠脉病变,术中注意探查冠状动脉情况,制订合适的外科治疗方案,可以显著提高Ⅰ型主动脉夹层的治疗效果.
Objective Summary surgical treatment experiences for type I aortic dissection combined with coronary artery disease. Methods There were 207 cases of type I aortic dissection from September 2002 to Febru- ary 2011. 73 cases combined coronary artery disease. These cases included 20 cases of coronary artery bypass grafting, 45 cases of coronary artery wall Integration and 8cases of coronary angioplasty. Results 2 patients died. The mortality rate was 2.74%. The long-term effect was satisfactory. Conclusion The patients which have type I aortic dissection combined coronary artery disease should be examined carefully before surgery. The surgeons explored coronary artery and laid down right surgery program. These measures can significantly improve the thera- peutic effect of type I aortic dissection combined with coronary artery disease.
