血清AFP、CEA、CA-199、CA-50联合检测对消化道恶性肿瘤的筛查价值 被引量:14

Diagnostic value of the combined detection of serum AFP, CEA, CA-199 and CA-50 in malignant tumors in digestive tract

摘要 目的联合检测血清中AFP、CEA、CA-199、CA-50的含量,探索此种筛查组合对常见消化道恶性肿瘤的应用价值。方法设立消化道恶性肿瘤组(216例)、消化道良性病例组(50例)和健康对照组(50例),运用化学发光法检测血清中AFP、CEA、CA-199、CA-50四种肿瘤标志物的含量,回顾分析其结果。结果 AFP主要对肝癌敏感,检出率为57.1%。CEA主要对结直肠癌敏感,检出率为40.7%。CA-199主要对胰腺癌敏感,检出率为67.9%。CA-50普遍对各种消化道恶性肿瘤敏感;与消化道良性疾病组及健康对照组相比,差异有统计学意义(P<0.05)。各个肿瘤标志物单独检测时敏感性均较低,而在联合检测时能显著提高检测的敏感性,与消化道良性疾病组及健康对照组比较,差异有统计学意义(P<0.05)。结论联合检测血清中AFP、CEA、CA-199及CA-50的含量,可以有效提高常见消化道恶性肿瘤的检出率,减少漏诊率,对消化道肿瘤的早期诊断及预防具有重要意义。 Objective To investigate the value of combined detection of serum alpha fetoprotein (AFP), carcinoembryonic antigen (CEA), carbohydrate antigen 199 (CA-199) and carbohydrate antigen 50 (CA-50) in the diagnosis of malignant tumors in digestive tract. Methods 216 patients with malignant tumors in digestive tract were selected as group A, 50 patients with benign tumors in digestive tract were selected as group B, and 50 healthy individuals were selected as group C. The levels of four tumor markers (AFP, CEA, CA-199 and CA-50) were detected and compared between the three groups. Results AFP was mainly sensitive to liver cancer, with the detection rate of 57.1%. CEA was mainly sensitive to colorectal cancer and the detection rate was 40.7%. CA-199 was mainly sensitive to pancreatic carcinoma and the detection rate was 67.9%. CA-50 was generally sensitive to malignant tumors in digestive tract. The sensitivity of combined detection of tumor markers was higher. The detection rate showed statistically significant difference between group A, group B and group C (P〈0.05). Conclusion Combined detection of serum AFP, CEA, CA-199 and CA-50 could effectively improve the detection rate, and reduce the rate of missed diagnosis, which has great significance in the early diagnosis and prevention of malignant tumors in digestive tract.
