摘要
目的 通过对女性乳腺疾病进行流行病学调查,了解女性乳腺疾病患病率及发病相关因素,制定乳腺疾病防治措施.方法 采用多阶段随机整群抽样的方法,抽取北京市海淀区4 952例35~69岁妇女作为调查对象,由调查员一对一填写调查问卷;专职乳腺科医生进行临床触诊、乳腺X钼靶检查、乳腺超声检查,可疑者进一步做组织病理检查.结果 调查对象中,患乳腺疾病1 755例,患病率35.44%,其中乳腺增生1 648例,患病率33.28%;乳腺良性肿瘤93例,患病率1.88%;乳腺癌11例,患病率222.13/10万.多因素回归分析显示：年龄＜40岁、月收入小于1 000元、较低文化程度是乳腺疾病的保护因素;较高文化程度、有乳腺疾病良性病史、城市居民是乳腺疾病的危险因素.结论 关注具有乳腺疾病危险因素的人群,大力提倡定期乳腺疾病普查,开展乳腺疾病防治的健康教育,降低乳腺疾病患病率.
Objective To investigate the prevalence of female breast diseases and related factors so as to work out prevention measures by carrying out epidemiological survey. Methods Multi-stage random cluster sampling method was used to collect 4 952 women aged 35-69 in Haidian District as the objects of the survey. Investigators completed the questionnaires one to one, and clinical breast palpation, breast X Mo target examination and breast ultrasonography were conducted by professional breast specialist. Pathological examination was made for suspicious manifestations. Results There were 1 755 cases of breast diseases, and the prevalence rate was 35.44%. Among them there were 1 648 cases, 93 cases and 11 cases of breast hyperplasia, breast benign tumor and breast cancer respectively, and the prevalence of them was 33.28%, 1.88% and 222.13/10 million respectively. Multivariate regression analysis showed that age 〈40 years old, income lower than 1 000 yuan one month and lower education were the protective factors of breast diseases, and higher education, history of benign breast diseases and urban residents were the risk factors. Conclusion Close attention should be paid to the crowd with risk factors of breast diseases and regular breast disease screening should be advocated. Meanwhile, health education on prevention and treatment of breast diseases need to be carried out to reduce the prevalence.
