摘要
目的:了解四川省重点慢性病病人建档与管理现状,为制定相关政策和措施推动慢性病建档及管理工作提供依据。方法:分析2008-2010年四川省基层医疗卫生机构开展高血压、糖尿病、重性精神疾病患者建档管理数量、质量和健康管理率、规范管理率等指标变化情况。结果:四川省2008-2010年全省重点慢性病病人建档管理分别为77.7万、93.2万、217万,对患者实施干预项目数分别为4、7、9项,规范管理患者分别为48.1、71、169.7万;乡镇卫生院提供管理服务的数量增幅高于社区卫生服务中心,但后者管理质量提高速度快于前者;经济基础较好的扩权县、工作起步早的市州管理质量、数量综合排名靠前。结论:2008-2010年四川省重点慢性病病人管理工作呈现较好的发展趋势,但仍需加大服务人群覆盖的范围,加强对档案的信息化管理,增强对基层医疗卫生机构和专业公共卫生机构的能力建设和政策投入。
Objective：To learn about the current patients filing and management status of major chronic diseases in Sichuan province and to provide evidence for developing relevant policies and measures to advance the patients filing and management of major chronic diseases. Methods：To analyze the change of patients filing quantity, quality, health management rate, and standard- ized management rate of hypertension, diabetes, and holergasia diseases that are carried out by primary health care institutions in Sichuan from 2008 to 2010. Results：The basic health care management numbers of major chronic diseases patients from 2008 to 2010 were 0. 777,0. 932 and 2. 17 million, respectively. The average numbers of intervention items are 4,7 and 9, respectively. The standardized management numbers of those were O. 481,0. 71 and 1. 697 million. The management service increase rate of township hospitals was higher than that of community health centers,whereas the quality of the latter is improved faster than the former. The overall management quality and quantity index of the reinforced counties with good foundation and the cities that star- ted earlier are ranking higher. Conclusions：The trend of this work is good in Sichuan from 2008 to 2010. It is necessary to extend the coverage, to enhance the information-based management, to improve the ability development and policy support of primary health care institutions and professional public health institution.
