摘要
目的探讨沙盘游戏治疗脑瘫儿童心理问题的疗效。方法采用随机对照研究方法,选取在儿童医院康复中心诊断为脑瘫的3~5岁儿童,以皮勃迪图片词汇测试(peabody picture vocabulary test,PPVT)排除精神发育迟滞儿童,最终选取20例脑瘫儿童为研究对象。将这些儿童采用区组随机法分为试验组(10例)与对照组(10例),试验组在常规康复训练的同时进行为期12周的沙盘游戏治疗,对照组仅进行常规康复治疗。以Conners父母症状问卷(parent symptom ques-tionnaire,PSQ)为测评工具,分别在试验前后对患儿进行疗效对照和评定。在治疗前后对患儿家长各进行一次半结构式访谈。结果①治疗后试验组品行问题、学习问题、冲动多动、焦虑、多动指数得分均较治疗前有明显降低(P<0.01)。②治疗后试验组品行问题、学习问题、冲动多动、焦虑、多动指数得分较对照组有明显降低(P<0.05,P<0.01)。③对患儿家长的半结构式访谈前后对比,有明显的好转。结论沙盘游戏能够有效解决脑瘫儿童的心理问题,对脑瘫儿童的心理康复有积极作用。
Objective To determine the therapeutic effects of sandplay therapy to solve psychological problems in cerebral palsy children.Methods A randomized control study was implemented in our center during September 2010 to January 2011.Children with mental retardation were excluded through Peabody Picture Vocabulary Test(PPVT),and finally 20 identified cerebral palsy patients at age of 3 to 5 were enrolled in this study.These children were randomly divided into the experimental group(n=10) and the control group(n=10).Another 20 matched normal children served as normal control.The experimental group underwent a 12-week sandplay treatment and simultaneously accepted the routine rehabilitation training.While the control group only accepted the routine rehabilitation treatment.The comparison and assessment of the treatment effects were carried out before and after the trial respectively.The Conners Parent Symptom Questionnaire(PSQ) was used as a measurement tool during the process.The semi-structured interview for their parents was also conducted separately before and after treatment.Results After the sandplay treatment,children's behavioral problems,learning problems,impulsivity and hyperactivity,anxiety,and hyperactivity index scores in the experimental group were obviously lower than before the treatment(P0.05).In the experimental group,the behavioral problems,learning problems,impulsivity and hyperactivity,anxiety,and hyperactivity index scores were obviously lower than in the control group after the treatment(P0.05).The comparisons of the semi-structured interviews for parents were significantly improved in the experimental group.Conclusion Sandplay therapy effectively solve the psychological problems in the cerebral palsy children,exerting positive effects for psychological rehabilitation of cerebral palsy children.
